10742-95 StreetEdmonton, AB T5H 2C9Tel: (780) 497-7146Fax: (780) 497-7156Team Leader – Assertive Community Treatment TeamBoyle McCauley Health Centre’s Pathways to Housing Program in Edmonton ends homelessnessfor people who suffer from serious, persistent mental illness (such as schizophrenia and bi-polardisorder), usually co-occurring with addictions and medical and social issues, by:

  • providing access to permanent independent housing without requiring treatment or sobriety as
a precondition for housing; and
  • providing outreach community support and treatment with a philosophy that is founded in the
principles of consumer choice and harm reduction, using both evidence-based best practicesand innovative approaches.As our Team Leader, you would report to the Program Director and provide leadership andmentorship to an inter-disciplinary Assertive Community Treatment (ACT) team that:
  • provides comprehensive wrap-around treatment, support and rehabilitation/recovery services;
  • uses a team case management model to form treatment/recovery plans using a person-centred
approach; and
  • provides assistance with housing, life skills and tasks of daily living, mostly within the Clients’
homes.Your Duties as the Team Leader
  • Provide leadership to the inter-disciplinary team to ensure that service delivery is consistent with
fidelity to our program model and principles.
  • Ensure each Client has a recorded strengths-based assessment, treatment and recovery plan that
is monitored to ensure effectiveness in supporting Clients toward their recovery goals.
  • Manage the Intake/Referral process, and relationships with referral agents.
  • Actively develop solid working relationships with allied agencies and services within the
community.
  • Spend approximately 30% of his or her time in direct Client service delivery.
  • Orientate, organize, support, coach, mentor and evaluate staff reporting to the Team Leader.
  • Participate as a member of the Clinical Leadership Team, to ensure standards of clinical practice
are appropriately maintained.
  • Actively participate in and supports BMHC’s organizational priorities.
  • Perform other duties as assigned.
Qualifications
  • Master’s Degree in Social Work, Psychology, Nursing, or related field. An equivalent
combination of work experience and education will be considered.
  • Demonstrated leadership, mentorship and staff supervisory skills.
  • Significant experience in the following areas:
o acute psychiatry and crisis intervention/emergency services;o mental health services in a community treatment setting;o Assertive Community Treatment or other inter-disciplinary teamwork;o working in addictions, concurrent disorders and/or Housing First; and/oro providing outreach services in a ‘street/inner city/chronic homelessness’ environment orequivalent.
  • Knowledge of best practices in
o working with people with complex co-occurring serious mental illness, addiction andmedical issues, including Assertive Community Treatment;o client centered, strength-based recovery treatments/rehabilitation strategies;o Housing First/Pathway to Housing models;o Harm Reduction philosophy.
  • Knowledge and practice in integrating cultural contexts into practice and service delivery.
  • Responsible, organized and efficient; solid communication/literacy skills (written, oral,
computer/technology)
  • A continuous learner
  • Valid driver’s license (non-GDL), use of personal car, and physical capacity to perform job duties
(including climbing stairs, carrying groceries, etc.) are all required for this position.This is a permanent, full-time managerial position. Regular hours of work are 8:30am-4:30pm, Mondayto Friday.
