Job Description

As the Care Manager, you will be responsible for providing effective leadership and strategic direction to an interdisciplinary team of healthcare professionals in the Home Care and Long Term Care departments in the delivery of safe and high-quality patient and family-centered care services. You will be making key decisions, providing clinical guidance and support as well as coordinating staffing, oversee meeting Continuing Care accountabilities, in alignment with AHS vision, mission, and business plan. Moreover, you will be responsible for facilitating improvement initiatives, fostering and maintaining dynamic and engaging teamwork and collaborative relationship with key stakeholders. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.Active registration with applicable Alberta Regulated Health Profession. Minimum of five (5) years of related experience in clinical practice required and within the last three (3) in a leadership role. Previous experience in Continuing Care required. Equivalences of education and experience may be considered.Proven clinical and leadership skills. Superior organizational skills. Evidence of strong leadership, interpersonal and communication skills. Evidence of initiative, flexibility, and creativity. Ability to establish and maintain effective relationships with patients, families, physicians, community agencies, professional colleagues, volunteers, and the community. Evidence of strong commitment to excellence in patient care and Continuing Care accountabilities. Ability to function effectively in a dynamic work environment.Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice will be considered. Previous management experience preferred.
