Specifically, this service supports those with COVID-19 symptoms greater than 12 weeks after initial diagnosis. The team is currently seeking a part-time physician to join this innovative, collaborative and interdisciplinary team of health professionals. Located in Fall River, this 0.5FTE position will provide comprehensive assessment and care planning for individuals with their Post COVID medical needs.The Post COVID Team is embedded in the long standing Integrated Chronic Care Service (ICCS). The ICCS model is based on the premise that chronic disease outcomes are better managed by addressing the needs of the whole person with a focus on self-management support. With a team of interdisciplinary health care professionals, we integrate the physical, psychosocial and environmental needs of individuals by focusing on the person instead of the disease(s)/condition(s), improving functional health and quality of life. The Post COVID team includes: Post COVID Navigator, Occupational Therapist, Physiotherapist, Clinical Therapist as well as ad hoc supports from the existing ICCS team.For more details about this service, interested physicians are encouraged to explore our website, myCOVIDRecoveryNS.caThis position is a one year term, 0.5FTE and is compensated through a sessional funding compensation model of approximately $6,500 per month. The position offers a competitive overhead rate. Scheduling is flexible, but physicians are expected to have regular weekly office hours to support team-based care and support care continuity Physicians in this role are expected to provide 70% clinical care, and 30% research activities.
