Using a collaborative, patient-centered approach to understanding how patients currently navigate transitions in care, the research aims to improve outcomes and lead to sustainable changes in the health care system. This position supports the work of the Strengthening Transitions in Care Lab and the Nova Scotia Applied Research Chair in Quality and Patient Safety and will report to Dr. Janet Curran and Dr. Christine Cassidy.Responsibilities include, but are not limited to:
