Job Description

Care ManagerAbout You You are an experienced Registered Practical Nurse or Registered Nurse that either has experience or would like to step to a management position.As a Care Manager with Stay at Home Nursing you would manage a portfolio of clients and ensure they have detailed care plans to achieve the goals they are seeking, and work to supervise the care givers providing that care to ensure positive client health outcomes and overall satisfaction. In your role your primary responsibility will be measured on the health outcomes and quality of care for your clients, and you will work closely with your scheduling team to ensure that there is an optimal match of the right caregiver (skills and personality) for your clients, and with Human Resources to ensure you have the right mix and quality of caregivers in your region to deliver the care required.   • Create detailed and individualized Care Assessments for every client, and ensure these are kept up to date over time
  • Ensures that every new Caregiver going into your clients has a First Visit briefing or introduction
  • Conducts supervisory visits to ensure Caregiver fit and quality of care
  • Communicates and coordinates with the client’s family and extended health care team to ensure continuity of care
  • Educates clients on self-care and evaluates client response to care provided
  • Monitors risk issues and analyzes trends, adapts care as needed
  • Coordinates care, implementing clinical solutions, and evaluates client outcomes based on client goals
Caregiver Leadership
  • Drives results through leading your caregiving team by setting clear goals, assigning tasks, coaching in the moment, and providing continuing education
  • Leading the client experience, ensuring our Caregivers are doing what it takes to provide high quality care and satisfaction to our clients and their families
  • Accountable for staff performance and proper following of policy in all client situations.
  • Manages the education function to support a continuous learning environment and staff utilization of learning opportunities. This includes Onboarding Day and monthly in-service programs
  • Conducts supervisory visits to evaluate Caregiver performance and job satisfaction
Scheduling Collaboration
  • Hands off accurate and timely client files to the Scheduling team to deliver exceptional client service by achieving scheduling goals and responding to urgent inquiries
  • Liaises with the Scheduling team to communicate changes in client requirements, Caregiver assignments
  • Responsible for ensuring client coverage is secured, particularly due to urgent care, caregiver cancellations and limited availability; assists with scheduling as needed
  • Responsible to ensure clock-in/clock out percentages remain below 5% for total staff; manages performance of Caregiver
  • Responsible for data integrity and completion (Client filesamp; caregiver documentation on performance) in the scheduling software
Consult Conversion
  • Conducts Caring Consults, assesses Clients with respect to physical, emotional and environmental needs and advises on Client care; achieves conversion goal of 80%
  • Drives business interest by engaging in relationships with community leaders and professional organizations to establish long term relationships that will translate into word of mouth and professional referrals
  • Develops, promotes, and ensures high quality, innovative customer service
  • Acts as advisor in Caring Consult to ensure that Client and Client contacts have all information required
Administration
  • Maintains excellent documentation practices, specifically, complete and accurate care plans, employee files (skillsamp; qualifications), client files, including pay and billing information
  • Reviews and updates client records quarterly to ensure for 100% accuracy and completeness
  • Directs the maintenance of client records to meet employee, organization and statutory requirements and entitlements
  • Conducts record audits on a quarterly basis to ensure accuracy and up to date information
  • Ensures compliance with national, state/provincial legislative requirements in areas such as health, employment, occupational healthamp; safety
Business Relationships
  • Reports to the Regional Director
  • Leads the Caregiving team on your clients
  • Works closely in partnership with Scheduling team
Qualifications
  • Education Requirements: Bachelor’s Degree
  • Nursing background required (RN or RPN)
  • Reliable vehicle and a valid driver’s license
Experience Requirements
  • 1-2 years of experience in nursing (preferred)
  • Strong computer skills, proficient in Office 365: outlook, drive, docs, calendar
  • Exceptional written and verbal communication skills
  • Intentional organization, planning, time management and multi-tasking skills
  • Demonstrated experience setting and achieving goals
  • Customer service and leadership background considered an asset
Job Type: Full-time Salary: $52,000.00-$65,000.00 per year RPN Benefits:

Schedule:
  • Monday- Friday 9am-5pm or 8am to 4pm
  • On call evening/weekends/holidays as needed (typically one evening per week, and once every 4-6 weeks for weekend coverage, 1-2 holiday days per year)
