The position is responsible for providing leadership to the planning, delivery and evaluation ofservices.In accordance with Northern Health’s vision where people and their families receive primary care servicessupported by interprofessional teams, the Manager of Community Services ensures the use of bestpractice to provide competent, safe, and ethical care for people and their families in settings such asphysician practices, homes, schools, and communities.The Manager, Community Services is responsible for the delivery, planning, organizing and evaluation ofservices for the assigned portfolio. This includes the overall management of operating systems, clinicalactivities, education and quality improvement in a variety of service areas such as: Primary CareInterprofessional Teams, Mental Health and Substance Abuse, Home Care Nursing, CommunityRehabilitation, and Home Support Services.QualificationsEducation and Experience

  • Baccalaureate degree in a related discipline.
  • Five (5) to seven (7) years of progressive leadership experience in direct team-based health services
or an equivalent combination of education, training and experience.
  • Operational experience in Primary Care, including a broad understanding of clinical theory and
practice within a team based, person-centered model of care.Knowledge, Skills and Abilities:
  • Knowledge of local and regional community health issues including a clear understanding of a Primary Health Care Model of service delivery.
  • Creates an atmosphere of respect, trust, and workplace health.
