Job Description

GENERAL DESCRIPTION:The Family Preservation and Youth Transition Workers are a full-time contract position. The Family Preservation and Youth Transition Worker is responsible for conducting assessments and planning services for on-going open protection and prevention cases, where children have returned to their home. The Family Preservation and Youth Transition workers develop, implement and evaluate a Plan of Support with the family to reduce the level of risk enabling children to remain with their family and community. The Family Preservation and Youth Transition Workers report to the Service Navigator.QUALIFICATIONS:

  • Experience working with youth and families;
  • Knowledge of the Child and Family Services Act and Regulations, Customary Care and the philosophy of service delivery, First Nation communities, family structure, and local customs and traditions;
  • Working knowledge of the administrative structure and operation of KFS including policies, procedures, guidelines, and protocols;
  • Knowledge of case management;
  • Knowledge of and experience in Traditional and Mainstream practices, and experience working with traditional Elders;
  • Knowledge of human behavior theories with the ability to apply in assessment and planning;
  • Excellent planning skills with proven ability to set, implement, and monitor service plan goals and objectives to mitigate risk and develop strategies for both reunification and prevention;
  • Ability to adhere to standards of family support delivery, including areas of the mental health act, confidentiality, disclosures, mandated reporting and any and all requirements of the Family Preservation program;
  • Excellent communication, organizational, time management, and interpersonal skills;
  • Committed to ensure the highest level of confidentiality;
  • Must possess a valid Ontario Drivers’ License and be willing to travel;
  • Must provide a clear Vulnerable Sector Check and Driver’s Abstract; and
  • Ability to speak Ojibway is preferred and a definite asset.
