Canadian Mental Health Association (CMHA) Kenora BranchKenora Supportive Housing ProgramKenora Supportive Housing Case Coordinator Full-Time PositionCONTRACT POSITION: 16 MONTHSSummaryThe Kenora Supportive Housing Case Coordinator provides direct leadership to a team of professionals whose focus is safety, stability, and support for individuals at the Kenora Supportive Housing operated through CMHA Kenora Branch. The Kenora Supportive Housing Case Coordinator provides case management and wrap around services for clients residing within the Kenora Supportive Housing Program. This is done through coordination of intakes, ongoing assessment, planning, coordinating, and connecting and supporting clients within the program. The Team Leader supervises and evaluates the Kenora Supportive with appropriate support to ensure excellence in the delivery of respectful, courteous, and professional service. Functioning as a practicing clinician on the team, the Kenora Supportive Housing Case Coordinator must be registered or licensed in one of the regulated health professions. The Kenora Supportive Housing Case Coordinator reports to and is responsible to the Chief Executive Officer of Canadian Mental Health Association, Kenora Branch.   • Adherence to all policies of the CMHA Kenora Branch
  • Assurance of confidentiality of all clients, personnel and organizational records and information
  • Maintenance of client files and data files including updating of client information consistent with policies and procedures of the CMHA, Kenora Branch
  • Ensure completion of clinical assessment tools (VI-SPDAT, OCAN Assessment etc.) within the program
  • Ensure completion of Individualized Recovery Planning and strategies to support the achievement of goals and objectives of clients within the Kenora Supportive Housing Program
  • Conducting regular formal or informal evaluation of client satisfaction, achievement of goals, reassessment of service requirements and or needs in consultation with the client
  • Participation in training and educational opportunities and application of new skills to practice
  • Provide crisis intervention, advocacy and education as indicated and ensure clients are connected to appropriate emergency supports
  • Provide case management and wrap around services to clients residing within the program
  • Provide mental health supports as required by the clients and their needs
  • Advocacy or linking of clients with appropriate services, supports and resources to meet the goals and objectives of the recovery plan
  • Liaise with key community stakeholders and partners around transition of residents from the program
  • Function as a fully integrated member of the team for the client residing within the residence
  • Participates and contributes toward the effective working of the team and overall operation of the agency
  • Monitor house rules compliance and report any issues to the Chief Executive Officer
  • Interface with community services and programs with and on behalf of residents
  • Facilitate and monitor the implementation of client programs for the residents, including daily programming and groups
  • Design and facilitate programming to enhance clients’ life skills (i.e., budgeting, healthy lifestyle, food preparation, household upkeep, personal safety)
  • Organize, facilitate, and participate in monthly staff meetings and other treatment plan meetings as deemed appropriate in relation to the residents
  • Organize and facilitate information sharing sessions with clients regarding topics of interest they have identified in their recovery plans
  • Promotes the Kenora Supportive Housing Program in various public events and by presenting program information to groups and organizations at community education events
  • Participates in the evaluation of the program operational goals and make recommendations on changes to current program activities for improved program delivery
  • Ensure all file management and reporting functions are maintained in a comprehensive and timely manner
  • Participate on specific Committees, tasks or work groups as directed (e.g., Program Advisory Committee)
  • Provide a safe environment by ensuring adherences to Workplace Health and Safety Regulations and
  • Develops policies and procedures at the direction of the Chief Executive Officer for the on-going functioning of the program
  • Provides support and supervision to agency volunteers and students assigned to the program area
  • Modify working hours as needed to meet programming needs of the residents
  • Drafting all necessary correspondence and reports regarding clients and Team Lead reports
  • Participation in administrative and office functions, including ongoing maintenance of the Kenora Supportive Housing Program
  • Ensure safety and security of all clients of the Kenora Supportive Housing Program during shift, including interpersonal safety and safety of Kenora Supportive Housing Program staff
  • General administration duties
  • Prepare Kenora Supportive Housing Program schedule and maintain scheduling program (approving time off requests etc.)
  • Communicate changes and new rules/regulations to staff; ensure all operation and program policies and procedures are being followed
  • Coordinate and facilitate orientation of the Kenora Supportive Housing Program staff
  • Participate in recruitment of new staff
  • Conduct staff supervision, performance review and team meetings
  • Attend education/training opportunities as required
  • Continue to build relationships within the District through one-on-on networking, group meetings, and public outreach as directed by Chief Executive Director
  • Participate on specific committees or working groups as assigned by Chief Executive Officer
  • Provide a safe environment by ensuring adherences to Workplace Health and Safety Regulations and Policies, Infection Control Guidelines, WHIMIS and Safe Work Procedures; including but not limited to Fire, Disaster and Evacuation Plans
  • Ensure client safety and client safety activities are emphasized throughout orientation of new staff members
  • Support an environment which avoids, prevents, and corrects all activities or actions which may result in an adverse outcome in the delivery of mental health services
  • Responsible for self-care
