Job Description

Are you interested in joining our care team whose mission is to improve the health of our customers in their home and community St. Anthonys Clinic Pharmacy in Langford, BC, is seeking Health Care Aides to join our ‘Heart @ Home’ team that offers mobile care services to our clients in their home. We strive to create a rewarding and dynamic work environment for all members of our team while providing loving care to our customers and community.We are looking to welcome Health Care Assistants with casual, part time, and full time availability.Position Summary:Visiting our Heart @ Home patients throughout the community at various times in the day.Mobile care that provides assistance with patient medication regime and administration (po pills, patches, application of creams).Routine Vital Signs per care plan ~ BP / Pulse / 02 Sats.Performs other related HCA duties as required.Documentation of interventions and assessment / Submitting daily reports to the pharmacy.Contributes to the development and modification of the patient/resident care plan.Work in collaboration with, and under the direction of, Heart@Home Program Manager & Nurse Manager; LPN’s and Pharmacists.Relay patient concerns and information to appropriate members of the health care team.Skills and Abilities:Enthusiastic, friendly & caring attitude, motivated to provide excellent customer care.Ability to communicate effectively, both verbally and in writing.Demonstrated ability to organize work and to deal effectively with others.Strong time management skillsAble to work independently with minimal supervision.Physically able to perform the duties of the job.Ability to effectively use a computer, electronic devices, and related equipment.Current CPR-C Basic Life Saving Certification (preferred).Requirements and Qualifications:Graduation from a recognized Nursing Assistant Program such as a Health Aide program or equivalent.Registered with the BC Care Aide & Community Health Worker Registry.Criminal record check will be required following an offer of employment.Valid BC Drivers License, clear Drivers Abstract and a reliable vehicle (mileage reimbursement included in compensation package).Proof of two COVID-19 vaccinations.What we offer:Competitive pay.Flexible schedule, no night or evening shifts required.Mileage reimbursement.Extended Health and Dental for full time employees.Access to an Employee Assistance Program, including counseling services, for all employees.Quick Apply
