Job Description

Behavioural Health ConsultantPermanent Part-time 0.6 FTEThe Leduc Beaumont Devon Primary Care Network is a joint venture between participating local family physicians and Alberta Health Services. Our mandate is to sustain and enhance the quality, coordination, and integration of health care services in Leduc and area and to improve the quality of life for the community of patients and physicians.Working as a member of the Team-Based Care Program, the Behavioural Health Consultant (BHC) position provides access to behavioural and emotional support to improve the health of PCN patients. Program emphasis will be on healthy living, behavioural interventions, biopsychosocial health education, emotional wellness and self-management support. The position will also provide crisis assessment, support and intervention as directed by PCN member physicians.Key Responsibilities:

  • Initial assessment on patients referred by PCN physicians and other team members to identify health concerns, current risk and readiness for behaviour change.
  • Utilize behavior modification strategies to assist patients with goal setting.
  • Employ health promotion and health education strategies to support health behaviour change.
  • Prepare and present individualized lifestyle education to clients and families.
  • Develop and deliver group psychoeducation classes.
  • Coordinate services and care with patients including facilitating referrals to appropriate service providers and social supports.
Hours of work will be dependent upon location but generally scheduled between 8 a.m. to 9 p.m. Monday to Friday.Qualifications:
  • Bachelor’s degree in Social Work, Psychiatric Nursing, Nursing or other related profession.
  • Active registration with an appropriate professional college or association that is recognized by the Alberta Health Professions Act.
  • Previous experience could include working in a primary care setting, group facilitation or instruction, using health behaviour change strategies e.g. motivational interviewing.
  • Ability to make professionally autonomous decisions in coordinating patient care.
  • Experience working with multi-disciplinary teams.
  • Strong effective written and oral communication skills.
  • Current criminal record check.
  • You must be fully immunized against COVID.
  • Must have own vehicle and valid driver’s license.
