The Ross family takes pride in providing compassionate, quality patient care.Department/Unit/Program DescriptionWho we are:Kawartha Lakes Ontario Health Team (KL-OHT) is a fully designated Ontario Health Team for the City of Kawartha Lakes and surrounding communities. Our mission is to coordinate and connect care to create a seamless experience for patients and families. By putting the patient at the centre of our system, we aim to reduce confusion, support more efficient navigation between care settings, and help keep people healthier, for longer.As part of the KL-OHT, you will have a unique opportunity to work alongside a dynamic team of highly-committed patient/client partners, care providers and leaders from across the continuum of care, and will work to pioneer a new approach to health care and related services.The Virtual Care Program Coordinator (VCPC) plays a lead role in the development of the Virtual Care Program (VCP) for the Kawartha Lakes Ontario Health Team (KL-OHT). The objectives of the VCP are to provide patients/clients with more choice in how and when they navigate the system and access their care; improve access to care in the most appropriate setting, including facilitating transitions in care; enhancing care for priority populations; and evaluating innovative approaches that improve the patient experience and provider efficiency.The incumbent will be responsible for the clinical, technical, and ongoing program development requirements for the VCP. The Coordinator will be responsible for the providing virtual care navigation for the patients/clients who will be participating in the program as well as increasing the overall capacity for delivery of virtual programming across the KL-OHT region. The Coordinator will facilitate IT set-up in patient/client home and support ‘warm transitions’ when possible.This position reports to the KL-OHT Executive Director and works extensively in collaboration with the RMH IT Director, and other KL-OHT staff and partners. As a member of the KL-OHT, Ross Memorial Hospital (RMH) is supporting the recruitment for this role. Responsibilities:
  • Works with KL-OHT Members to on-board patients from Year 1 target population – ensuring where possible and appropriate that program attachment is made within 7 days post hospital discharge
  • On boards applicable CCCKL clients – may include establishing connectivity and installing devices in clients’ homes.
  • Recruits and trains up to 5 virtual volunteers in collaboration with CCCKL
  • Provides care navigation within the scope of the proposal
  • Engages other providers and/or locates and connects consenting patients/clients to appropriate virtual services (Alzheimer’s Society, CNIB, etc.)
  • Works with collaborative of health service providers to identify best practices in virtual programming for older adults across a range of domains from chronic disease self-management to social recreation and grief support
  • Works with KL-OHT to identify ongoing opportunities for expansion of virtual programming across the OHT region
  • Enhances capacity of clients to be full partners in their access and utilization of health care services through supporting utilization of the emerging digital health patient portal for KL-OHT with attributed population
  • Ongoing data collection and analysis to inform program efficacy and development.
Essential Qualifications
  • Graduate of an accredited College or University or equivalent recent experience
  • Regulated Health Professional – member in good standing of respective College
  • Valid Drivers License and access to a vehicle for visits to clients homes
Minimum of 1-2 years of experience with project management, coordination with multiple stakeholders
  • Demonstrated experience with Virtual Care Tools and patient information systems
  • Knowledge of all legislation and guidelines associated with Health Information and Patient Registration
  • Experience with document analysis and interpretation
  • Demonstrated experience working with multidisciplinary teams to execute and drive improvement opportunities
  • Excellent customer service skills
  • Demonstrated Microsoft Office capabilities – strong Excel skills required
  • Demonstrated Apple and iOS skills
  • Ability to work independently and in a team environment
  • Excellent communication skills – oral, written and non-verbal
  • Excellent interpersonal skills
  • Sound judgment, tact, initiative and creativity in planning
  • Organizational skills
  • Analytical/critical thinking skills
  • Attention to detail
  • Must be able to fulfill physical requirements of the job, as per the current Physical Demands Analysis
Preferred Qualifications
  • Certified Professional in Healthcare Information and Management Systems – Canada (CPHIMS-CA) would be an asset
  • Project Management designation or equivalent experience is considered an asset
  • Change Management designation or equivalent experience is considered an asset
Hours Of WorkMust be available for 37.5 hours per week.
