The CSFM’s research is based on the principles of patient-centered care, and takes place in three contexts: clinical care; the community; and the health care system.As a primary care clinician researcher within CSFM, my program of research follows the idea that “…practice needs to be research informed, and research needs to be practice informed.” (Herbert, Carol). Diabetes and obesity are now recognized to be two of the most serious health epidemics of the 21st century, with an estimated 80% of patients with diabetes being cared for by primary healthcare practitioners. It is imperative that we continue our efforts towards these patient populations, as well as provide guidance to primary care practitioners caring for these patients.ResponsibilitiesThe Project Coordinator role will support the implementation of the LIBERATE project (LIBre Enabled Reduction of A1c Through Effective Eating and Exercise), which aims to “operationalize” lifestyle as a form of medicine (exercise and nutrition). The program will use behavioural coaching techniques and flash glucose monitoring technology (FreeStyle Libre) in patients with Type 2 Diabetes attending Canadian Diabetes Education (DECs) or Management Centres (DEMs). The Project Coordinator will support the design, coordination and operationalization of the research project, and support the development and execution of aerobic and resistance exercise programs for subjects. The role will coordinate the successful implementation of the project, and ensure that the research outcomes are achieved.QualificationsEducation:
