Reporting to the Supervisor of our youth community justice programs, the Service Coordinator will serve as the primary point of contact for the satellite site, supporting youth aged 12-17 at the time of their charge(s) and their families/support system, as they navigate the justice system. Youth will attend first appearance court dates onsite and meet with the Service Coordinator to complete an informal assessment, be connected with and supported through warm referrals to internal and external resources, and to ensure ongoing case management support. This role requires a strong ability to engage youth, collaborate with justice system professionals, and coordinate services to ensure youth have timely access to meaningful supports within their community.Key Responsibilities:Youth Engagement and Case Development

• Assist youth and their supports in attending their first court appearance at the satellite site, supporting with systems navigation, explaining the court process, and ongoing engagement throughout their involvement within the satellite Justice Centre
• Conduct informal assessments to understand the youth’s strengths, risks, and needs, as well as that of the family and/or support network
• Develop individualized service plans in collaboration with youth and their supports to set meaningful goals and connect them to services within their community
• Conduct outreach as needed to connect youth and their supports in services
• Provide crisis intervention, de-escalation, and advocacy as needed

Collaboration with Justiceamp; Community Partners

• Be present at the satellite site to support youth in court and liaise with justice professionals
• Work closely with justice stakeholders including Judges, Crown Attorneys, Duty Counsel, defense lawyers, and probation officers to advocate and collaborate for youth-centered interventions
• Participate in partner and stakeholder meetings, including with the Ministry of Children, Community and Social Services (MCCSS) and the Ministry of the Attorney General (MAG) as required
• Participate in community outreach and system navigation efforts to improve service accessibility for youth and their supports
• Develop and strengthen relationships with schools, child welfare agencies, hospitals, probation, ethno-specific organizations, and other community-based supports to provide wraparound services

Service Navigation

• Maintain an extensive referral network, connecting youth to internal and external supports, including mental health services, housing, education, employment programs, and restorative justice initiatives
• Ensure youth and their supports are involved in decision-making regarding service plans and community connections at all times

Administration and Professional Development

• Support team in implementing the systems navigation model within the daily operations and provide feedback and coaching as necessary
• Model and support the team in the use of crisis prevention, intervention, and other de-escalation strategies as needed
• Maintain accurate documentation in compliance with agency policies and funder requirements, including case notes, service tracking, and data collection, using the case management system and other tracking methodology
• Actively participate in individual and team meetings, supervisory meetings, project planning, and designated training
• Support in program evaluation, funding reports, strategic planning, committees, and other organizational initiatives as required
• Other responsibilities as may be assigned

RequirementsWHO YOU AREEducation/Experience

• Post-secondary education in Social Sciences, Social Work, Criminology, Youth Justice, Community Development, or related field (equivalent experience will be considered)
• Minimum three years’ experience working with justice-involved youth in a case management or support role (related experience will be considered)
• Understanding of relevant legislation including working knowledge of the Youth Criminal Justice Act (YCJA), and the Child, Youth, and Family Services Act (CYFSA)
• Understanding of the implementation of the YCJA within GTA-based courts, including court and bail proceedings

Knowledge and Key Competencies

• Strong understanding of trauma-informed, anti-oppressive, and culturally responsive practices
• Ability to engage and build rapport quickly with diverse youth and their families/supports
• Strong crisis intervention and conflict resolution skills
• Excellent written and verbal communication skills, including documentation, case management, and report writing
• Experience in community engagement and developing collaborative partnerships with service providers, agencies, and stakeholders
• Experience with community-based research, program evaluation, and data analysis, as well as data tracking and reporting
• Direct experience engaging with diverse populations and an ability to apply a culturally responsive lens to all interactions, including program implementation and capacity-building with young people
• Satisfactory Broad Record Check (BRC) and Vulnerable Sector Check
• Ability to travel within the Greater Toronto Area (GTA) for meetings and community visits
• Flexibility to work evenings as needed to accommodate youth and their supports schedules
• A self-starter who can work independently while collaborating within a multidisciplinary team
• Proficiency in Microsoft Office and case management software

In addition, the following are considered assets:

• Understanding of relevant legislation and institutional policies (including the Education Act, Safe Schools Act, etc.)
• Knowledge of the social service agencies operating in Toronto, GTA, and provincially
• Strong network of youth and justice stakeholders across the province
• Current First Aid/CPR certification
• Crisis Prevention and Intervention certification (CPI)
• Mental Health First Aid
• Applied Suicide Intervention Skills Training (ASIST)
• Fluency in French or second language
