Job Description

We are proud to offer services on the traditional, ancestral, and unceded territories of the Katzie First Nation, Kwantlen First Nation, and the Coast Salish People including the territories of the Musqueam, Squamish, and Tsleil-Waututh Nations.Opportunity:We are looking for a full-time Outreach Family Preservation Worker for the Ridge Meadows area reporting to the Team Lead, Ridge Meadows. You must be able to travel to families’ homes and within their community to provide a variety of parenting education and supportive interventions that promote healthier family functioning, reduce risk factors, maintain safety and increase well-being.Cases range from seeing a family once per week for 8 to 12 weeks/6 months, to delivering intensive intervention support 5 days in a row for a two-week period.Job Responsibilities:

  • Conducting and completing detailed assessments for clients referred from the Ministry for Children and Family Development
  • Assessing, screening, referring and linking individuals to internal and community resources as needed
  • Compiling and gathering collateral information
  • Participating in Integrative Case Management meetings as needed to determine outcomes for individuals referred to the Agency
  • Consulting with the Ministry for Children and Family Development to co-ordinate and deliver services
  • Coordinates and consults on cases
  • Provide interventions and brief therapeutic supports to individuals experiencing crisis
  • Linking individuals with urgent needs to immediate resources (i.e. securing food resources, financial assistance, and housing)
Skills You Bring:
  • An understanding of Integrated Case Management.
  • Strong counselling, program planning, and communication skills.
  • Experience working with families who may present multiple service needs related to poverty, addictions, violence, child abuse or neglect, trauma and mental health issues.
  • Computer proficiency – MS Office, ERP/CRM software systems (e.g. Sohema)
  • A background in parent-education and family interventions
  • Experience supporting and counseling at-risk families with children ranging from infant, school-aged, and youth
  • Adept at providing client services in non-traditional environments, such as client homes setting
  • Ability to embrace a hybrid working model
  • Multilingualism an asset.
Requirements:
  • Relevant B.A. degree (i.e. Social Services, Counselling Psychology) with 2 years’ experience or relevant experience in family preservation/reunification preferred
  • Current Standard First Aid with CPR & AED Level C certificate (or obtain in 3 months)
  • Valid Driver’s License with satisfactory driving record and use of own vehicle.
  • Satisfactory Criminal Record Check.
