Job Description

What we are looking for:The NRS/CCRS coordinator is responsible for leading and coordination of accurate and efficient assessment and data entry under the both National Rehabilitation Reporting System (NRS) and Continuing Care Reporting System (CCRS) across Oak Valley Health. This role will ensure the completeness, accuracy, timeliness, compliance with Canadian Institute for Health Information (CIHI) guidelines, and submission of NRS and CCRS data.The NRS/CCRS coordinator is the key contact between Oak Valley Health and the CIHI concerning the dissemination of NRS and CCRS related information between the two organizations.The NRS/CCRS coordinator understands and prompts the key drivers of funding and performance measurement related to the NRS and CCRS data, and identifies and leads areas where further training or process improvement is required.Key Responsibilities:NRS/CCRS Coordinator
  • Be accountable for completeness, timeliness, submission and data quality of NRS/CCRS assessments at Oak Valley Health
  • Work with teams on the units to optimize processes to ensure timely, accurate and efficient completion of NRS/CCRS assessments as per required standards
  • Remain abreast of and communicate changes in reporting requirements, modify processes and implement changes as necessary
  • Review reports including outcomes, case mix and comparisons to peers to identify opportunities for improvement and implications
  • Summarise and present NRS/CCRS data in a meaningful way to both inform and empower the care teams and hospital leaders when required
  • Supports hospital leaders and clinical programs on understanding, interpreting, and properly utilizing NRS and CCRS data for funding, operations, clinical outcome, and performance measurement purposes
  • Identify needs and request appropriate modifications or custom reports from application team to enhance the process for frontline users - Participate in testing of software enhancements
  • Liaise with relevant external stakeholders (such as GTA Rehab Network, Accreditation Canadas Stroke Distinction program, Ontario Health) as required
Education:
  • Provide ongoing education and training for all NRS/CCRS assessors in Oak Valley Health ensuring all assessors are certified/re-certified as required
  • Maintain and revise all required educational materials and guidelines required for NRS/CCRS assessments
  • Identify the need for, and appropriately coordinate, in-house training, including access to online CIHI training to continually improve data quality
Auditing:
  • Ensure NRS/CCRS data quality by monitoring process and activities on the units and conducting data quality audits in collaboration with clinical programs and Decision Support
  • Based on audit results, provide recommendations to Clinical Directors/Patient Care Managers or clinical staff, re-train assessors, and update taring materials
What you bring to the role: Required:
  • Current license and member in good standing with an affiliated professional college
  • A Bachelors or a Master’s degree in a health related field (PT, OT, Nursing)
  • A Minimum of three years clinical experience in rehabilitation and/or complex continuing care inpatient setting
  • Proven and recent experience in working with NRS/CCRS data
  • Certification as a NRS Trainer or commitment to begin certification process within 6 months
  • Proficient use of MS Office suite (specifically Word, Excel, PowerPoint)
  • Display a positive attitude towards complex challenges as well as routine maintenance and administration tasks
  • Self-motivated and able to work independently with minimal supervision and also possessing a positive team spirited attitude
  • Strong planning, organizational, leadership and prioritization skills
  • Demonstrated interpersonal skills and the ability to build consensus among a wide variety of clinical and administration professionals
  • Strong written and verbal communication and presentation skills
  • Ability to multi-task with strong attention to detail within a fast-paced environment with changing priorities
  • Adhering to confidentiality and privacy requirements for sensitive financial and clinical information
  • Demonstrated ability to work effectively with enter-professional teams, and internal and external stakeholders
Considered an asset:
  • Certified in National Rehabilitation Reporting System (NRS) or Continuing Care Reporting System (CCRS) preferred
  • Recent experience in education or a leadership role
  • Expertise in the delivery of formal and informal education/coaching
  • Demonstrated Project Management experience and training
  • Data Analytics and reporting skills
  • Experience with performance measurement and quality improvement methodologies
  • Experience in leadership and/or education position
  • Prior experience with Meditech
NOTE:
