Are you ready to join usWhat we need:Working as an integral member of the inter-professional team, you will play a critical role in providing social work services to patients and their families within the organization, in the acute care setting, with the exception of in-patient mental health. There is a strong emphasis on psychosocial support, interdisciplinary problem solving, and liaising with appropriate members of the team to ensure patients and families are linked with necessary resources and supports. Within a fast paced environment you will be able to prioritize a very diverse caseload of patients and respond to crisis situations as they ariseThe successful candidate will demonstrate exceptional leadership skills, a high level of professional integrity and accountability and play a key role in system development, as well as possess a strong level of understanding of care across the continuum. Experience in short-term supportive counseling with patients and families dealing with difficult decisions and end of life concerns would be a valuable asset.Duties and Responsibilities will include:Completing psychosocial assessmentsShort-term, goal oriented support and counseling for patients and families dealing with grief, chronic life altering issues as well as end of life concernsWorking with the health care team, patient and family to develop patient care plansCross coverage for other Social Workers within the hospitalConsultation with community health care professionalsMaintaining up dated knowledge of community resourcesAttending and responding to gaps in service deliveryAttendance on committees and tasks forces as appropriateCompensation:BAND I: $39.46 - $50.27 per hourWork Schedule:The successful candidate will be available to work a variety of shifts, 0830 – 1630 Tuesday, Wednesday and Saturday . Successful candidate must be available to rotate though all sites. Please note that schedules may change due to operational needs.What you bring to MSH:Master of Social WorkRegistration with the Ontario College of Certified Social Workers and Social Services WorkersRecent related previous social work experience in a hospital environmentUnderstanding of loss/adjustment and the grief processDemonstrated strengths in interpersonal relations, problem solving, and conflict resolutionDemonstrated excellent communication skillsDemonstrated commitment to professional developmentExperience in working with electronic documentation, preferably, MeditechDemonstrated good attendance and performance records with the ability to maintain these same standardsDemonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as requiredMarkham Stouffville Hospital takes pride in serving some of Canada’s most diverse communities.
