Job Description

Job BriefWe are seeking a dedicated and compassionate Social Services Worker / Social Worker (12-month term) to join our team and make a positive impact on the lives of individuals in need. As a Social Services Worker, you will play a crucial role in assessing clients #39; needs, developing, and implementing care plans, and connecting them with the appropriate resources and support services at Phoenix Recovery Center in Moncton, NB area. Your empathy, strong communication skills, and commitment to social well-being will be essential in helping clients navigate challenging situations and improve their overall quality of life.Responsibilities:

  • Conduct thorough assessments of clients #39; social, emotional, and economic needs to identify areas requiring support and intervention.
  • Collaborate with clients and the multidisciplinary care team to develop individualized plans that address their specific circumstances and goals.
  • Provide counseling and guidance to clients on various social and personal issues, including mental health, family dynamics, housing, employment, and substance abuse.
  • Connect clients with community resources, government programs, and other service providers to ensure access to essential services such as healthcare, housing assistance, food programs, and legal aid.
  • Regular communication with external stakeholders such as Probation Services, Correctional Services of Canada, Social Development, and others.
  • Maintain clinical records/documentation monthly for submission to Mental Health Team and management.
  • Stay informed about changes in social service policies, regulations, and resources to provide clients with accurate and relevant information.
  • Participate in training sessions, workshops, and professional development opportunities to enhance your skills and knowledge in the field.
  • Use of Microsoft Word, Excel, Outlook, Adobe Acrobat, and other medical related computer software as required.
  • Contribute to team, department and company goals by accomplishing related work as assigned.
Qualifications:
  • Post-Secondary Education in Social Services, Human Services, Psychology, or a related field from an accredited community college or university.
  • Social Work designation considered an asset.
  • Experience in social services, case management or a related field.
  • Strong interpersonal skills and the ability to establish rapport with a diverse range of clients.
  • Must be detailed oriented to ensure accurate deliverables with ability to take initiative to meet deadlines.
  • Excellent communication skills, both written and verbal, for effective client interactions and documentation.
  • Empathetic, non-judgmental, and culturally sensitive approach to working with clients from diverse backgrounds.
  • Problem-solving abilities and the capacity to work independently as well as part of a team.
  • Familiarity with legal and ethical guidelines for social services work.
  • Positive attitude and the ability to reflect a professional company image.
