Job Description

The Social Worker holds a leadership position in our Park Place Home. You will take lead with the interdisciplinary team through all resident admission processes while also providing guidance and comfort to new and present residents along with their families. This integral role provides our residents and their families with professional services by developing, implementing and monitoring programs which meet the social, emotional, spiritual and intellectual needs of the residents.The Social Work role ensures a commitment to resident safety and to our culture of safety as per the Strategic Directions of Park Place Seniors Living. Providing assessment, care planning and treatment coordination for residents while working collaboratively with families and the multidisciplinary team, you will provide direct psychosocial care, counseling, system navigation and advocacy, education and the pathway to relevant resources. Scheduling Information:Shift times are from 08:00 to 16:00 hours. Days of work are Monday to Thursday.How Will You Create ImpactSocial Workers help to advocate for social justice at individual, family, organizational, and societal levels through their professional knowledge, values, ethics and skills. Joining our MLSC team will allow you to challenge yourself professionally while enjoying all the personal pursuits available to you within this fantastic community.Education, Training and Experience:- Bachelor’s Degree in Social Work from an accredited university.- Two years related experience in a long-term care setting working with medically complex, impaired residents and those with Dementia. It would be beneficial if knowledge and understanding of Public Trustee and Guardianship roles/agencies.
