Job Description

TITLE OF POSITION: SCHEDULING COORDINATORTITLE OF IMMEDIATE SUPERVISOR: NURSE CARE MANAGERSUMMARYThe scheduling coordinator performs clerical functions to ensure the efficient processing of referred clients to Qualicare Newmarket Family Home Care. She/he organizes and schedules referred clients and anticipates staffing requirements to effectively manage client volumes. The activities are conducted in accordance with the organizations policies and procedures; may be required to be on call after regular business hours as directed by the Nurse Care Coordinator.ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:Responsibilities of the Scheduling Coordinator include, but are not limited to the following:Scheduling

  • Responds to clients referrals within specific time frames as per the organizations policy;
  • Effectively meets the needs of each client by scheduling the client with the caregiving services;
  • Identifies team availability and ensures equitable distribution of client assignment while ensuring adequate coverage at all times.
  • Assigns clients to the site within their geographical area to provide cost effective, time efficient service.
  • Monitors referral trends and anticipates increases/decreases;
Documentation/Communication
  • Responds to client/employee inquiries and/or problems immediately, as appropriate refers to the Care Manager/Supervisor for necessary follow-up and action;
  • Ensures all relevant information is received either in verbal/written/electronic format.
  • Inputs accurate data into the Qualicares software package;
  • Assists with tracking and validating client related documentation;
  • Uses technology to facilitate accurate and timely communication i.e. voice mail, email
On-Call Requirements
  • Rotate on-call responsibilities with Care Manager, one week on-call every two weeks.
  • Serves as a first point of contact for all employees and clients after hours
  • Communicates and problem solves schedule change needs in a timely manner
  • After-hours intake coordinator and scheduler; enter referrals into the system. Answer any urgent calls through Clarity or our answering service
Quality Management:
  • Responds promptly to incident/complaint/compliment, documenting completely and relays information to Care Manager/Supervisor immediately;
  • Integrates quality criteria and standards into daily work practice and participates through suggestions, actions, etc. in continuous quality improvements in all aspects of operations;
  • Assesses client satisfaction using Qualicares established process
Community Relations:
  • Is aware of community resources and ensures implementation of appropriate information.
  • With the approval of the Care Manager, attends workshops and seminars to further own knowledge as well as establish business contacts.
  • Promotes the services in the community
PROFESSIONAL DEVELOPMENT REQUIREMENTS
  • Minimum Grade 12 education
  • Minimum of two (2) years office experience preferably in the health care field.
  • Keyboarding accuracy and knowledgeable in Windows environment.
  • Bilingual (English/French) an asset.
  • Work days include Holidays and Weekends
