Job Description

SummaryAnchor SILP Case Managers work as a team that is responsible for directly supporting residents at The Lighthouse with an intellectual disability in areas of daily living support, behaviour support, and companionship. The goal of the Anchor SILP team is to address the gap in service for residents experiencing intellectual disability who struggle with chronic challenges related to stable housing, mental health and substance use disorders, and/or high risk lifestyles by providing residential support that is rooted in person-centredness, trauma informed practices, and harm reduction strategies.Core Competencies

Job Duties· Assertively engage with residents with an intellectual disability who are a part of the Anchor SILP program· Participate in the development of support strategies that are person-centred and based on the needs and wishes of the resident that set them up for success· With direction from the Anchor SILP Coordinator, collaborate with resident and existing support system to develop a Person-Centred Plan· Assist resident with all daily living activities including room care, laundry, grocery shopping, meal planning, meal preparation, booking and attending appointments, etc.· Identify and facilitate access to community services, community activities, and outings for resident· Provide companionship and coaching opportunities to residents to develop skills in areas of daily living, health management, hygiene, and money management· Maintains all required daily documentation, tracking and monthly reporting· Present self in an approachable, friendly and professional manner as an ethical example of a professional community organization· Become familiar with and adhere to Lighthouse Policies and Procedures· Participate in ongoing team development, training, and team meetings· Maintain knowledge and skills in areas of mental illness, addictions, homelessness, trauma informed practice, crisis management, harm reduction strategies, person-centredness and positive behaviour support· Work collaboratively with other departments and program areas as needed· Perform other duties as assignedRequirements· Disability Support Worker Certificate, or Degree/Diploma level of training in Human Services (Addictions, Nursing, Social Work, Psychology) preferred, along with professional registration where required, or equivalent combination of experience and training· Experience developing and facilitating programs for individuals with disabilities· Have strong values in person-centredness, trauma informed care and harm reduction strategies· Understands the need to adapt approach and communication with people with disabilities in order to meet their needs effectively when necessary· Strong commitment to providing unconditional resident care· Knowledge of resources related to the disability sector· Knowledge/understanding of poverty, homelessness, addiction and mental illness· Good organizational, time management and prioritizing skills.· Leads by example and exhibits strong professionalism· Demonstrated ability to work in a team environment· Excellent interpersonal and communication (written and verbal) skills with other departments in The Lighthouse and external community partners· Proven critical thinking and decision making skills.· Strong crisis management and de-escalation skills· Computer literacy, including effective working knowledge of MS Office, E-mail and Information System applications.· Physically able to perform all assigned tasks· Ability to perform all duties in accordance with related legislation· Criminal Record and Vulnerability Sector clearance required as a condition of employment· Valid Class 5 Driver’s LicenseWork Conditions
  • Hours of work are flexible; evenings and weekends required
  • May be exposed to infectious waste, diseases and conditions.
  • Interacts with residents, staff, partners, as well as government/community agencies and personnel.
  • Manual dexterity required to use computer and peripherals.
  • Physical activity required including walking, standing, sitting, climbing stairs, cleaning duties, as well as working alongside of residents to coach life skills, effective self-care and activities related to tenancy.
  • Intermittently assisting care aide and other staff in the conduct of their roles.
