Job Description

Personal Support Worker, HAL/APC(1 Full-Time Vacancy)CCD is looking for a full time Personal Support Worker to join our Home at Last and Access to Primary Care teams located throughout Durham Region. Under the direction of the Program Manager, the PSWs serve as relief and a resource for caregivers and families caring for a loved one. The HAL/APC program provides settlement for clients coming home from the hospital. This includes providing appointment support that is possibly outside of the Durham Region. The PSWs specifically address the needs of high-risk seniors who can reside in their own homes, PSW tasks may include but are not limited to:

  • Personal Support Services including dressing, personal hygiene, assisting with mobility, assisting and monitoring medication use and other routine activities of daily living
  • Homemaking services including shopping, housecleaning, and meal preparation
  • Safety checks and/or reassurance services
  • Transition assistance to clients, including transportation from hospital and settlement services at the home
  • Completing necessary client/visit documentation and reports
Minimum Requirements:
  • Personal Support Worker certificate (minimum of 600 hours in duration, including in class and practicum experience), or acceptable equivalent in education (e.g., RN or RPN, or 2nd year nursing student) and experience in the community setting
  • Current Standard/Emergency First Aid with Level C CPR
  • Satisfactory Police Vulnerable Sector Check or Enhanced Police Information Check
  • Current immunization record (TB test completed within the past 12 months) is required upon hire
  • Adherence to COVID-19 safety practices including two doses of COVID-19 Vaccine (where medically able in accordance with our Immunization Policy)
  • Comfortable working alone in the community (with After Hours Support)
  • Sensitivity to the needs of the frail elderly and persons with disabilities, including challenges in cognition
  • Physical ability sufficient to perform client transfers and activities of daily living and able to lift approximately 50 lbs., bend, stretch, stand for extended periods of time, climb stairs, reach, twist, sit, walk, and/or run
  • Pushing clients up/down ramp in wheelchair accessible van
  • Be able to access clients’ readiness for discharge at hospital (1 person assist, level of mobility) Willing to be trained on driving wheelchair accessible van & HAL van with Bruno seat
  • Comfortable working with clients with low mobility range and/or varying levels of mobility
  • Comfortable with directly assisting clients in medical appts such as taking notes, assisting with making follow-up appts., & requests’ as per family members
  • Flexibility with hours within a set schedule
  • Licensed vehicle and the ability and willingness to drive within Durham Region (including, but not limited to Ajax – Oshawa, Bowmanville, Uxbridge, Cannington & Blackstock
  • Willingness to drive to Scarborough and/or Downtown Hospitals for client pick-ups as well as Orillia and Lindsay etc.
  • Maintenance of class G Ontario driving license and clean Drivers Abstract with personal automobile insurance of at least $2,000,000 and appropriate business-use coverage as required by personal automobile insurance carrier
  • Providing proof of 3 years driving experience with clients would be considered an asset
Compensation: Starting at $21.50 per hour; plus the additional $3 temporary wage enhancement; 3 weeks’ vacation per year of work; 11 paid (including stat) holidays; Group Health & Insurance Benefits, Healthcare Spending Account, and Pension/Retirement Savings Plan.
