The APSW provides enhanced case management services including customized supports to assist in the areas of crisis resolution, criminal justice, health, housing and employment to adults living independently who have a developmental disability within Canopy’s catchment area including Peterborough, Haliburton, Kawartha Pine Ridge and Northumberland area. The APSW fosters growth and independence skills related necessary for daily living, and help them enhance their support network, awareness of generic community based resources, government-funded services and supports.Primary Responsibilities:Assisting individuals to identify goals and priorities and supporting them to develop case management plans using a strength based approach with the goal of supporting them to attain the maximum independence possibleAssessing the individual’s capacity in activities of daily living, accessing supports to address areas of need and supporting them to develop needed skillsSafeguard the rights of individuals either by advocating on their behalf with their agreement, or encouraging and providing opportunities for individuals to learn how to become self-advocatesParticipate in case consultations, court appearances, interagency committees and other community initiativesQualifications:Bachelor Degree in Social Sciences or related social services field or equivalent3+ years’ experience case management or short-term consultation experienceKnowledge of relevant legislation and community resources along with a developed networkStrong conflict resolution and organizational skillsAbility to work flexible hoursComputer proficiency and excellent communication skillsCompassionate and responsible supporter of people, their families, and community partnersCommitment to maximize resources and improve people’s quality of lifeA valid driver’s license and insurance, access to a reliable vehicle and ability to drive in various weather conditionsWe look forward to having a person who shares our passion and commitment to developmental services join us!How You Can Join Our Team:If you’re interested in working with us to better your community, then please click Apply now to complete your application!We will review applications as they are received until we find our successful candidate.
