Job Description

Director of Clinical Informatics- HOPE STREET TERRACECandidates are invited to apply for the Director of Informatics, full time, permanent position of (DOI) at Hope Street Terrace; a 77 bed accredited long term care facility located in Port Hope, Ontario.Reporting to the Executive Director (E.D), the DOI is accountable for assisting with the management of all aspects of nursing care within the facility to ensure quality resident care through the effective, efficient delivery of established standards within legislative and budgetary requirements.RESPONSIBILITIES:

  • Works in partnership with the DOC in the implementation and evaluation of the nursing department quality improvement initiatives, risk management systems, data collection and analysis, report writing, procedural updates,
  • Takes the lead in the coordination and implementation of best practice and risk management initiatives related to resident care (i.e. Falls, Restraint use, Wound Management, Infection Control, Responsive Expressions etc),
  • Ensures risk management process is adhered to by reviewing staff and resident incident reports, monthly indicators, Public Health Inspections, Ministry of Labour inspections, Accreditation reports, etc.,
  • Will be the designated professional to provide IPAC support in the absence of IPAC lead.
  • Maintains a system of care assessment, planning, implementation and evaluation at the resident and unit program level that maximizes staff resources and contributes to Southbridge interdisciplinary team approach to resident care,
  • Delegates clinical and managerial responsibilities as well as provides direction and leadership,
  • Conducts and oversees the RAI-MDS process and ensures each resident has a RAI-MDS assessment completed on admission, quarterly and at a significant change of condition’
  • Monitors all CIHI indicators and the Case Mix Index for the Home,
  • Assists with the development of succession plans and identify employees with growth potential for development, as well as completing Performance Appraisals as needed.
  • Oversees family communication through Town Halls, telephone and email correspondence,
  • Effectively manages utilization of nursing supplies,
  • Maintains an active presence in the Resident Home Areas mentoring staff while overseeing the provision of quality resident care,
  • Works collaboratively with the Director of Care on special projects, reviews, and other duties as may be assigned,
  • Delegation of an authorized act procedure to unregulated care providers (Personal Support Workers),
  • Oversees nursing staff based on clearly defined lines of authority and communication, performance management, participate in recruitment and selection process, conduct workplace investigations, and provide coaching, as well as disciplinary action when needed,
  • Recommends and participates in the termination of nursing staff according to facility policies, procedures and collective agreements,
  • Initiates investigative and corrective actions related to occupational health and safety; develops and monitors modified work programs for nursing personnel,
  • Implements goals and objectives to achieve the corporate vision for resident care,
  • Ensures appropriate documentation and maintenance of records including resident health records, personnel records etc.,
  • Review potential admissions in HCCSS,
  • Participates in the accreditation process,
  • On call expectation on a rotational basis,
  • Assumes responsibility for the nursing department in the Director of Care’s absence,
  • Maintains Legislative Compliance with all appropriate legislation especially the FLTCHA and Regulation 79/10.
QUALIFICATIONS:
  • Registered Practical Nurse (RPN)
  • Registration with College of Nurses of Ontario
  • Minimum of three years experience in long term care; previous management experience an asset
  • Current knowledge of clinical issues for geriatric residents, infection control, principles of adult education and quality improvement
  • Knowledge of RAI MDS
  • Demonstrated superior leadership and organizational skills; excellent verbal and written communication skills
  • Computer skills: Microsoft Word; Excel, computerized resident care and assessment programs
  • Knowledge of applicable legislation, current nursing and healthcare practices; demonstrates concern and interest in providing quality care and service to residents
