They will lead and improve system-level processes for discharge planning of all adult acute care at HSN while prioritizing and enabling improvement work within the system by increasing value of discharge planning to patients and families.The successful candidate must have a minimum of a Bachelor’s Degree with a specialization in a health-related field from an accredited university. Preference will be given to candidates with advanced training in lean management and continuous quality improvement. A minimum of five (5) years’ experience working in an acute care environment with a minimum of two (2) years’ experience working in a leadership/supervisory or process improvement role. Prioritize and enable improvement work within the system by increasing value of discharge planning to patients and families.REPORTING:Under the general direction of the Administrative Director, Care Transitions, Patient Flow and Utilization.DUTIES:

  • Engage HSN staff, leaders and physicians in mapping current discharge planning processes and sub-stream processes (e.g. Discharge to Long Term Care (LTC), Slow-Paced Rehabilitation (SPR), Palliative, Transitional Care, and processes related to the designation, review and flow of Alternate Level of Care (ALC) patients).
  • Work with relevant stakeholders to identify and prioritize opportunities to further design, develop and/or improve future state processes and standards of work related to discharge planning and ALC avoidance.
  • Coordinate the activities of improvement teams, identify appropriate resources needed, and develop schedules to ensure timely completion of improvement work.
  • Establish processes/audits to monitor compliance of standards of work and future state critical processes.
  • Identify opportunities for improving patient transition from HSN to other care facilities/agencies through collaborative discharge planning.
  • Manage daily operations of Utilization office, including staff resources, policies and procedures.
  • Work with ALC Prevention Committee to identify opportunities for ALC avoidance; lead the implementation of prioritized initiatives.
  • Monitor the day-to-day activities within the discharge planning value stream to ensure improvements are implemented.
  • Measure the impact (e.g. value creation, waste reduction) of improvements during the transition to the future state value stream; report key performance indicators to stakeholders.
  • Analyze data regarding emerging trends and recommend resource allocation to address shifts in the presenting populations.
  • Promote early identification of barriers to achieving improvement outcomes, and engage in four step problem-solving at the unit, program, organizational and system level.
  • Ensure improvement work is patient-focused while engaging patients and families as key partners in system re-design; evaluate patient care and satisfaction and formulate solutions for improvement.
  • Champion the adoption of evidence-informed discharge planning and best practices related to ALC prevention.
  • Work collaboratively with Access and Flow Management to help manage day-to-day activities and patient flow within the program.
  • Design, promote, and facilitate the use of systems to ensure provision of patient care is consistent with program and professional standards.
  • Develop, coordinate, and maintain staff schedule/rotation for Amberwood Alternative Hospital Site.
  • Ensure program delivery is consistent with regulations, legislation, professional practice, and evidence-based best practices.
  • Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  • Participate in the accreditation process and work to ensure that the program and HSN achieve, maintain and continually.
  • Teach, coach and lead staff in the development and monitoring of quality indicators and improvement initiatives.
  • Engage in and foster professional development of all staff and provide opportunities for learning within budgeted resources.
  • Ensure a safe environment for patients, staff and visitors; investigate report, debrief and take or direct corrective action as required on incidents.
  • Collaborate with internal and external stakeholders/organizations/community groups to achieve program objectives, integrate and link services and foster partnerships across the continuum of service delivery.
  • Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  • Represent the department or program on various committees and in meetings as required.
  • Perform other duties as required.
QUALIFICATIONSEDUCATION AND TRAINING:
  • Minimum of a Bachelor’s Degree with a specialization in a health-related field from an accredited university.
  • Advanced training in lean management and continuous quality improvement is preferred.
  • Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
  • Minimum of five (5) years’ experience working in an acute care environment.
  • Minimum of two (2) years’ experience working in a leadership/supervisory or process improvement role.
  • Experience working with multidisciplinary teams, physicians, patients and families to improve outcomes.
  • Experience working with external partners for systemic improvement.
KNOWLEDGE/SKILLS/ABILITIES:
  • Demonstrated knowledge of health research and/or best practice guidelines in discharge planning and ALC avoidance.
  • Demonstrated superior knowledge of evidence-informed approaches related to discharge planning, ALC avoidance and improved utilization of acute care services.
  • Demonstrated time management, prioritization, organization, and problem-solving skills.
  • Demonstrated knowledge of relevant provincial legislation topical to discharge planning (i.e. consent, capacity, privacy, etc.).
  • Demonstrated understanding of key system constraints based on analysis of data and trends (i.e. bottlenecks, delays, resource challenges, gaps, etc.).
  • Demonstrated exemplary leadership skills, motivating and influencing individuals involved in the value stream to remain focused on creating value for our patients, and reducing waste for our system.
  • Demonstrated effective presentation skills.
  • Demonstrated evidence of building trust with patients, employees, medical staff and leaders.
  • Demonstrated ability to use tact and discretion in dealing with health care providers and employees.
  • Demonstrated ability to coach, advise and teach others using the principles of adult learning.
  • Demonstrated training, experience or utilization of lean methodology for process improvement.
  • Demonstrated knowledge of relevant legislation and principles of management, hospital committee structure, conflict management and resolution, system needs analysis and organizational assessment.
  • Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  • Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  • Demonstrated superior interpersonal and communication skills, both written and verbal.
  • Demonstrated commitment to the safety of co-workers and patients.
