Your Opportunity:The Rockyview Day Hospital is an alternative to inpatient admission within Addiction and Mental Health acute care services. This four-week program operates on a stabilization model and supports patients in enhancing coping skills. The Mental Health Clinician (MHC) is a key member of the patient #39;s treatment team and will work as a primary therapist and case manager with assigned patients within the Day Hospital. The MHC will be involved in a range of clinical activities related to planning, implementing, and evaluating interventions with patients including initial and ongoing patient assessment and intervention, individual and group facilitation, and facilitation of discharge planning. You will be responsible for the intake, clinical interviews, and screening of clients with concurrent addiction and mental health disorders. You will liaise with a variety of internal and external stakeholders and work collaboratively with the multidisciplinary team in the delivery of care for the needs of individuals, families and/or communities. In addition, you will be responsible for timely documentation of clinical activities, clinical data entry and regular participation in clinical consultation, team education and ongoing program planning, development, and evaluation. You may be required to provide supervision to practicum students. A demonstrated commitment to collaborative practice to enhance client health outcomes; ability to set realistic expectations and boundaries in the therapeutic relationship; strong interpersonal and communication (written and verbal) skills; ability to effectively manage change; ability to work autonomously and collaboratively in a multidisciplinary team; crisis management; and knowledge of psychotherapeutic modalities are requirements of this role.Description:As a Mental Health Clinician, you will work as a member of the multidisciplinary health care team collaborating on the delivery of care for the needs of individuals, families and/or communities.
