Objectives for this RoleA community service worker will augment the efforts of the pharmacist, and the addictions worker to provide a whole-person approach to care of clients by acting as a liaison person for the pharmacy. The community service worker will create engagement and partnership by adding value beyond the charitable support the pharmacy is already providing to organizations such asFriendship Inn, The Bridge on 20th Fellowship Centre, and various community health projects run by nursing students. The worker will help to expand the partnership and scope of health,physical, psychological, and social support to the increasing population of vulnerable individuals within the community. The growing population of vulnerable individuals include First Nations,Immigrants, homeless persons, individuals with special needs, the elderly and people with substance abuse and addictions problems. The expanded partnership will provide the synergy to necessary for more productivity, efficacy, and deliverables. The partnership will include CYNC,Light House. YWCA, The Salvation Army, Food Bank, Sasks Abilities, and Immigrant ServiceAgencies. This role will consolidate the efforts of the community pharmacy to serve, consolidate, and impact the lives of clients served to produce measurable deliverables.Duties and Responsibilities
