Job Title: Community Health Worker II (Health Care Assistant / Care Aide)Work Site: Home Support, Sunshine Coast - Gibsons/Sechelt, BCStatus: Regular Full TimeStart Date:As soon as possibleSalary: $23.27 - $25.45 per hourHours: Hours and days of work vary as operationally required.Reference#: 145382-eagCome work as a Community Health Worker with Vancouver Coastal Health (VCH) in Sechelt, BC!Community Health Workers are a core part of our communities as caregivers and make a significant impact on the quality of life of their clients and patients.As a Community Health Worker II at Vancouver Coastal Health you will make your impact by:Providing home support services to clients while working under the direction of the Supervisor/Coordinator/Manager or delegate.Assisting clients with the activities of daily living within varied settings to promote maximum independence.Observing and documenting on your clients’ health conditions and environment, reporting significant and unsafe observations and providing support and guidance.Providing input and feedback in the care planning process.Assisting in meeting the nutritional and elimination needs of clients.Performing additional delegated tasks under the Transfer of Function guidelines for clients in specific situations for which training has been provided.New grads of the Health Care Assistant program are welcome to apply!Community Health Workers at Vancouver Coastal Health operate within the context of a client and family centred care model and in accordance with established vision and values of the organization.ABOUT HOME SUPPORT-SUNSHINE COAST:Home Support provides a range of support options designed to help people with ongoing health conditions to remain in their homes and to be as independent and safe as possible. Home Care Teams provide case management and home care nursing services, with healthcare professionals who care for clients with kindness & dignity, while ensuring their safety and well-being.ABOUT SECHELT, BC:The District Municipality of Sechelt is located on the lower Sunshine Coast of British Columbia, approximately 50 km northwest of Vancouver. It is accessible from the mainland by a 40-minute ferry trip between Horseshoe Bay and Langdale, followed by a 25-minute drive along Highway 101, also known as the Sunshine Coast Highway. This charming seaside community is known for its natural beauty and is a popular destination for outdoor activities including sightseeing, boating, diving, camping and mountain biking. That is what Sechelt has to offer you!QUALIFICATIONS:Education & ExperienceGrade 10.Graduation from a recognized Home Support/Resident Care Aide program or an equivalent combination of education, training and experience.Registration with the BC Care Aide & Community Health Worker Registry.Valid BC Drivers License. Local travel requires the use of a personal vehicle.Knowledge & AbilitiesHome management skills.Physical ability to carry out the duties of the position.Ability to work independently and in cooperation with others.Ability to operate related equipment.Ability to communicate effectively both verbally in writing.Ability to organize and prioritize.Ability to observe and recognize changes in clients.Ability to establish and maintain rapport with clients.The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
