Visit us today and check out www.novascotia.com to see why more people from across the globe are moving here.ResponsibilitiesReporting to the Manager of Seniors (Western Zone), or designate, the incumbent is a member of the Seniors LINCS interdisciplinary team and participates in the planning, development and provision of Social Work services for seniors. The Social Worker works to full scope of practice as an independent practitioner within the team. This includes supporting the development and integration of seniors’ health services within health care and the community and providing collaborative, client centered care across the continuum, supporting the psychosocial and emotional well-being of all individuals (includes patients, families, groups, populations and communities). This position includes case finding activities and considerable community collaboration.The Social Worker ensures that the principles and practices that guide NSHA such as the mission, vision, values, expected behaviours, and the leadership philosophy along with organizational health, population health, ethics, safety, quality, partnerships and interprofessional collaboration are integrated within the services one provides and through the messages one delivers.Qualifications

  • Masters of Social Work required
  • Currently registered or eligible to be registered with the Nova Scotia Association of Social Workers.
  • Minimum two (2) years clinical experience.
  • The applicant would practice as an independent practitioner as a part of an interdisciplinary team. A high level of independent practice may require additional training and/or study in clinical practice sub-specialities regarding clinical practice.
  • Experience working with the geriatric population within a health sector environment is an asset.
  • Ongoing professional development participation is required.
  • Basic proficiency with Microsoft Office Suite (Word, Excel, Powerpoint, Outlook) and other electronic health programs such as Meditech, Medworxx, SEAscape etc.
  • Travel is required. Must hold a current NS Driver’s License and have a reliable vehicle.
  • Knowledge of both internal and external community resources
  • Proven leader with good decision making skills. Supervisory, teaching and/or community experience is an asset.
  • Successful applicants are required to provide a criminal reference check and vulnerable sector screen as a condition of employment and assume any costs associated with checks.
  • Demonstrated good attendance and work history in current and past employment.
  • Physical capabilities to perform the duties of the position.
  • Competencies in other languages an asset, French preferred.
