Job Description

Scope of Position:Under the supervision of the Registered Nurse and as a member of the multidisciplinary care team, to plan and supervise the daily personal care to the Residents using a person centered model, being aware of the whole persons’ emotional, physical, intellectual and spiritual needs. To assess, plan, implement and evaluate nursing care and service to the Residents, including the administration of prescribed medications and treatments, working in cooperation with the home physician(s) and the Health Care Team. Position Description:

  • To provide team leadership and direction to non-regulated nursing staff.
  • Deliver Resident Care using the nursing process (assessment, planning, implementation, evaluation)
  • To administer medications and treatments as prescribed and maintain the medication system (including relevant documentation).
  • To collaborate with the Resident, family, physician and Health Care Team regarding the Residents’ condition, plan of care and services required; and to evaluate the effectiveness of the care and services provided.
  • To participate in various teams / committees as required.
  • To participate as a team member in providing Resident centred care and service.
  • Undertake all duties and responsibilities in compliance with Health and Safety standards, guidelines and legislation.
  • Participate in in-service educational opportunities.
  • To orientate new staff as required.
  • Maintain awareness (at all times) of emergency procedures; participate in drills and associated educational opportunities.
  • To adhere to infection control policies, procedures, protocols, routines and additional practices as required.
  • To be aware of and comply with all Corporate and facility policies and procedures.
  • Confidentiality is of extreme importance and must always be maintained.
  • To perform any other duties as assigned.
RequirementsKnowledge and Experience:
  • Registered Practical Nurse with a current Certificate of Competence with the College of Nurses of Ontario (including medication administration course)
  • Over one year up to and including 2 years of previous experience in a long term care setting an asset.
  • Working knowledge of Ministry of Health and Long Term Care standards and guidelines and relevant legislation an asset
  • Courses specific to gerontology and dementia studies an asset;
  • Gentle Persuasive Approach Certificate would be considered an asset;
  • Supervisory experience an asset;
  • Resident Assessment Instrument, Minimum Data Set (RAI MDS) an asset;
  • Computer skills – experience with a computerized Resident Documentation system an asset; must be able to apply basic functions of Corporate standard software;

  • Proven organizational skills
  • Effective oral and written communication skills
  • Excellent interpersonal skills
  • Ability to facilitate and adapt to changes in the long term care environment
  • Current Basic Cardiac Life Support (BCLS) certification
  • Ability to perform Heimlich Manoeuvre
  • Resident and Customer focus
  • Ability to work independently as well as part of a team
  • Valid Ontario driver’s license and access to a reliable vehicle
  • Computer expertise in corporate standard software (Microsoft Office) and department specific software to data search and entry and to create and modify word processed documents
  • Must be aware of safe work practices as they relate to job responsibilities and work environment, and have the basic understanding of the Occupational Health and Safety Act
