Job PostingLet’s Go Home LeadCommunity Support Services of Niagara (CSSN) is currently seeking a full-time permanent (35 hours per week) Let’s Go Home Lead to add to our team. CSSN is a volunteer based, not-for-profit agency providing comprehensive support services to assist seniors and adults with disabilities to live independently in their homes. Reporting to the Program Manager, the successful candidate will be responsible for the implementation of the Let’s Go Home program.The Niagara LEGHO (Let’s Go Home program) is a provincial initiative targeting two population segments within seniors 65+ residing in Niagara. The goal of the initiative is to reduce hospital admissions and need for Alternative Level Care (ALC) in individuals 65+ living with dementia and reducing Emergency Department (ED) visits (and repeat visits) in individuals 65+ presenting to the ED with Canadian Triage Acuity Scale (CTAS) levels of 3-5 with non-acute/clinical diagnosis.Responsibilities:

  • As the Lead Agency, the LEGHO Lead will oversee all aspects of Let’s Go Home program.
  • Assists with the development and maintenance of policies, procedures, and resources to support LEGHO referrals, intake, system navigation, care coordination and follow up activities.
  • Works alongside colleagues to develop care plan and coordinate services for LEGHO clients.
  • Works collaboratively with LEGHO partners to maintain sufficient and/or appropriate levels of service capacity, and to monitor referrals and outline responsibilities of all partners.
  • Maintains regular contact with clients to ensure satisfaction with program.
  • Maintains accurate hardcopy and electronic client files and service volumes.
  • Oversees and manages partner invoices as they relate to the LEGHO program.
  • Manages client requests for on-going fee-based services upon the completion of the LEGHO six-week bundle program service.
  • Ensures clients understand the parameters of the LEGHO program.
  • Makes internal and external referrals for clients as needed and appropriate.
  • Completes program evaluation, including satisfaction surveys and pre-post evaluations for quality improvement purposes, ensuring alignment to province-wide monitoring and evaluation requirements.
  • Maintains confidentiality of client/agency information
  • Participates as a CSSN team member, promoting the agency and contributing to quality improvements
  • Attends meetings and training, as required.
  • Maintains current knowledge of community resources and services
  • Performs other duties as assigned.
Qualifications and Requirements:
  • Degree/diploma in social services or a relevant health care field
  • Previous experience working with volunteers
  • Excellent interpersonal and conflict resolution skills
  • Excellent written and oral communication skills and attention to detail
  • Excellent organizational skills
  • Ability to take initiative, work independently and as a team
  • Proficient computer skills – Microsoft Office, database management
  • Valid Ontario driver’s license and use of automobile is required.
  • Social media experience is an asset
  • Bilingual in French and English is preferred
