Job Description

Your Opportunity:The Continuing Care Counselor (CCC) provides psychosocial supports and services in both community and facility settings. Using a collaborative approach, the CCC supports clients, families, and health professionals across the continuum of care, such as Home Care, Supportive Living and Long Term Care. This support may include, but is not limited to, cognitive and mental health assessments, assistance with substitute decision-making, resource provision, and referral to appropriate supports within and external to Alberta Health Services.Description:As a Continuing Care Counsellor (CCC), you will be working closely with interdisciplinary professionals with a variety of backgrounds where you will provide a wide variety of coordination activities to support patients and their families to access Continuing Care living options. Using a collaborative approach, you will review applications, manage continuing care waitlists and coordinate placement processes. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures, while reflecting the shared vision and values of AHS.

Required Qualifications:Active or eligible for registration with applicable professional association or college.Additional Required Qualifications:Strong knowledge of applicable legislation (Personal Directives Act, Adult Guardianship and Trusteeship Act, etc), the health care system, and community resources are required as this position is a resource for the interdisciplinary team. Specific skills in Case Management, group facilitation, adult education, and adjustment/supportive counseling are assets. The client group served is diverse; clients may be adults of any age. Comfort and experience with a diverse population including those with acquired brain injuries, and mental health challenges and dementia and the behaviors that maybe associated to this population is required. Services to clients may be provided in the hospital, home setting, congregate living setting, shelters or care home setting.Preferred Qualifications:Very strong communication skills and the ability to function both independently and as a contributing member of the interdisciplinary skills both within AHS and external agencies are key to this role, as is the ability to balance multiple demands in dynamic environments.Quick Apply
