Job Description

We honour and strive to model through our actions the traditional Seven Teachings of the Anishinaabe: Love, Respect, Courage, Honesty, Wisdom, Humility & Truth.SUMMARY: The Case Aide provides administrative and participant support services under the general direction of the Clinical Case Manager (CCM), Heart to Home (H2H) Programs in The Link North’s Thompson and The Pas locations. The Case Aide provides support to the CCM in delivery of care plans, coordination of meetings, case reviews, home studies, administrative duties relating to licensing regulations and other duties as assigned. This role’s primary location may be in The Pas or Thompson with travel required to the other location, based on program and participant needs.Duties and responsibilities are carried out within a trauma and resiliency informed framework that promotes The Links’ Therapeutic Model of Care, including the agency Mission, Guiding Principles and the Circle of Courage® values. Duties are performed within a team environment that focuses on establishing collaborative and trusting relationships with one another and with agency participants. Providing culturally competent services and ensuring participant safety, choice and control are priorities at The Link. PRIMARY RESPONSIBILITIES:of the Case Aide include, but are not limited to:

  • Assists the CCMs with monitoring and re-evaluation of treatment plans on an ongoing and consistent basis (i.e. tracking task completion and other planning specific goals/activities);
  • Assists the CCMs with monitoring per diem expenditures, including the use of respite/support worker hours, mileage, clothing and activity funds;
  • Assists the CCM with transfer of learning plans according to position/department goals and specific participant treatment needs;
  • Schedules and coordinates family visits as per each participant’s treatment plan;
  • Supports the CCM to complete all necessary licensing processes for foster parents and respite workers;
  • Works in consultation with the other H2H staff, Foster Parents and internal stakeholders to ensure continuity and communication throughout the North’s H2H Programs;
  • Assists the CCM with the coordination of tasks identified in the discharge plan;
  • Works with The Link North’s Supervisors and Managers to ensure a smooth transition at time of participant discharge, including planning and transition to other North Programs.
  • Ensures appropriate and timely record keeping;
  • Attends team meetings, and children and youth care planning meetings;
  • Attends relevant training as required by agency policy or supervisor;
  • Liaises with The Link’s other Programs.
  • Attends mandatory agency and program events;
  • Represents The Link at agency and community functions;
  • Other duties, as required.
QUALIFICATIONS AND EXPERIENCE:The successful candidate should demonstrate the following competencies, which incorporate the Seven Sacred Teachings-Love, Respect, Courage, Honesty, Wisdom, Humility & Truth.
  • Degree or Diploma in applicable Human Services field preferred; A suitable combination of education and experience considered;
  • Knowledge of Indigenous cultural practices and experience working with Indigenous youth, agencies and community organizations considered an asset;
  • Knowledge of systems for children in care, school, family functioning and child welfare.
  • Demonstrated ability to build effective professional relationships and partnerships with internal and external stakeholders, agencies and organizations in the community;
  • Strong written and oral skills;
  • Microsoft Office at an Intermediate level;
  • Satisfactory Criminal Record Check with Vulnerable Sector Search (check must be completed through local law enforcement agency); valid if completed within the last 3 months;
  • Satisfactory Child Abuse Registry check with acceptable Prior Contact checks and a clear Adult Abuse Registry check, all done within the last 3 months;
  • Willingness to obtain training within three (3) months of employment, as per The Links policy and under licensing requirements, including but not limited to:
    • Valid First Aid/CPR C Certification preferred;
    • ASIST Certificate (Applied Suicide Intervention Skills Training) preferred;
    • NVCI Certificate (Non-Violent Crisis Intervention Training) preferred;
MANDATORY REQUIREMENTS:
  • For this role, you must have a valid 5F Drivers License and provide a current Drivers Abstract (subject to approval)
  • Ability to travel to/from The Link Norths locations throughout Thompson and The Pas, MB based on service needs;

In these cases, appropriate supporting documentation will be required.**SALARY RANGE: $36,080.00 - $54,120.00 per annum; depending on qualifications. This position is also entitled to a northern allowance equal to an additional 5% of the salary, and enrollment in The Links employee health benefits plan following successful completion of probationary period.LOCATION: Thompson and The Pas, ManitobaHOURS OF WORK: 35 hours per week; Current hours of work are Monday to Friday 8:30am to 4:30pm (subject to change based on service requirements); must be capable of working a variety of shifts, including days, evenings, weekends, and holidays.HOW TO APPLY: Please submit a cover letter and resume by the deadline.
