Job Description

The Diabetes Educator Dietitian is an integral member of a multi-disciplinary team, who will promote nutritional health within thecommunity. This will be achieved through expertise in diabetes, specifically in the six critical program areas of prevention,education, care, management, support and research. The Diabetes Educator Dietitian will create interdisciplinary collaborativerelationship with fellow health care team members, and other community agencies to ensure a high standard of practice anddelivery of quality client care.Guided by Northern Health Region Primary Health Care principles, this position will strive to improve the health status of theresidents of the Northern Health Region (NHR). Working from an advanced understanding and knowledge of Chronic DiseaseManagement, harm reduction, population health, community health development social determinants of health, and casemanagement delivered within a primary health care framework, the Diabetes Educator Dietitian will develop and implement healthplans from a client centered strength based/holistic approach.This position functions within the provisions of the Vision, Mission, Values, Policies and Procedures of the NHR.The incumbent must fulfill the requirements of the Criminal Records/ Vulnerable Person, Child Abuse Registry Check and AdultAbuse Registry Check, and adhere to all NHR policies and procedures.QUALIFICATIONS• Baccalaureate degree in Nutrition or Food Science or equivalent program from recognized University

  • Completion of Dietitians of Canada Accredited Dietetic Internship Program
  • Registration with the provincial Regulatory Body as a Registered Dietitian
  • To be an active Certified Diabetes Educator or to become a Certified Diabetes Educator within two (2) years of employment
  • Current certification from a recognized Healthcare Provider Basic Life Support (BLS), or obtain within three (3) months of
commencing employment
  • Must have a valid Manitoba Class V driver’s license and access to a vehicle and willingness to use it to travel in region
throughout the year
  • Current knowledge and experience in the areas of Social Determinants of Health, understanding the impacts these have on
sustainable behavior change and overall health outcomes
  • Two (2) years (recent) of directly related experience in working with clients with Diabetes or other chronic illnesses
preferred
  • Two (2) years of directly related work experience in community health and/or Primary Health Care Community settings
preferred
  • Satisfactory employment record required
  • Successful candidate subject to provide security checks in accordance with Security Checks for Employment Policy (HR-02-
