Schools Plus Facilitator - CC#21-06-10Schools Plus FacilitatorCompetition # 21-06-10Location: Schools in Celtic Family (Pictou County)One Year Term, Non-Union PositionSchoolsPlus provides preventative, proactive programming as well as interventions for students and families. The vision for SchoolsPlus is for schools to become centres of service delivery enabling enhanced collaboration by bringing professionals and programs together to help children, youth and families. SchoolsPlus promotes an integrated service delivery model through co-location and/or partnership of services within a school such as: Community Services, Justice, Mental Health, Addiction Services and other community organizations.Duties:Under the direction of the SchoolsPlus Lead, the SchoolsPlus Facilitator manages difficult and complex caseloads involving addictions, mental health, child welfare and justice issues; provides programming and support to youth at risk and their families; and develops and implements comprehensive service plans for youth with complex needs and then assesses their progress. The SchoolsPlus Facilitator maintains a monthly caseload, workload and other pertinent statistics for the purpose of both internal office and departmental evaluation and planning. They interview and seek input from school staff, service providers, students and families to determine ongoing needs of students and families and plans accordingly based on their social work knowledge, skills and values. They monitor, assess and evaluate progress by maintaining ongoing contact with students and families who are on the Schools Plus caseload. They coordinate and link students and families to support and services as needed based on their social work knowledge. They assist clients with problem solving for social, emotional, academic, behavioral and legal problems. The SchoolsPlus Facilitator liaises with regional centre personnel and outside agencies to increase access to services for identified students and their families. Also, coordinates programs and activities that support the SchoolsPlus model to achieve student success and develop and maintain a positive public image and effective community relations. They will be responsible for the direct supervision of the Community Outreach Workers and Child and Youth Care Practitioners within their respective area.Qualifications:Masters of Social Work plus two years related experience; or Bachelor of Social Work plus four years related experience.Experience in integrated service delivery and direct service delivery to children, youth and families with complex needs.Current registration with the appropriate regulatory body, or eligible to become registered.Familiarity and knowledge of the Regional Centre, Community Services, Justice, Health and Wellness and community resources.Capability to develop budgets and supervise funding utilization to ensure efficient and effective allocation of resources.Ability to keep accurate records, maintain confidentiality and create case plans.Experience working independently as well an ability to work collaboratively with various service providers.Demonstrated experience in group facilitation and working with Advisory Committees.Ability to engage and work with diverse communities and as part of a multi-disciplinary team.Excellent interpersonal, oral and written communication skills.Ability to plan and organize a demanding caseload with minimum supervision.Willing to work flexible hours and over the summer.Valid driver’s license and willingness to travel as needed.Interested applicants must apply electronically through this Online Careers page.Interested applicants must attach a résumé with a concise covering letter highlighting relevant qualifications and experience.
