The Social Worker will provide individual and family counseling; social work consultation and education services for patients admitted to hospital. The Social Worker will work with community systems at all levels, while recognizing each client’s right to self-determination. The Social Worker will provide psychosocial assessments, intervention and social work collaboration as a member of all acute care hospital based care teams and in our communities.
