Visit us today and check out www.novascotia.com to see why more people from across the globe are moving here.ResponsibilitiesReporting to the Manager Palliative Care, Seniors’ Clinic and Spiritual Care and working in collaboration with the Palliative Care Team, the Social Worker will provide services throughout Cumberland. This position provides a full range of social work services to clients and families, that encompass clinical social work; counseling; social work consultation and education services for Palliative Care clients and families in community setting. The Palliative Care Social Worker will act as an advocate for those with a life-limiting conditions assuring basic human needs are in place while respecting the client’s right to self-determination while upholding human rights. The Palliative Care Community Social Worker will be an active member of the collaborative inter-professional Palliative Care Consultation Team and will work in shared care with each client’s Primary Health Care Team.Qualifications

  • Master of Social Work (MSW) from an accredited university
  • Registered or eligible for registration with the Nova Scotia College of Social Workers (NSCSW)
  • Minimum two (2) years experience in hospital based Social Work practice is required
  • Experience working as part of a Palliative Care Team
  • Certification in relevant therapeutic modalities, as required
  • Completion of LEAP (Learning Essentials Approach to Palliative Care) preferred
  • Commitment to Community-based Palliative Care Approach
  • Commitment to the philosophy of person and family centered care; and primary health care
  • Demonstrated personal and professional leadership abilities
  • Proven advanced clinical knowledge and the ability to use sound professional judgement
  • Experience in facilitating education programs
  • Ability to work in a self-directed environment and also effectively within an interdisciplinary team
  • Working knowledge of a variety software and programs (Meditech, e-mail. MS office)
  • Valid driver’s license and reliable transportation with appropriate insurance.
  • Must be willing to work flexible hours and travel throughout zone, as needed
  • Excellent interpersonal, oral and written communication skills
  • Excellent organizational and prioritizing skills
  • Physical ability to perform duties of the position
  • Commitment to confidentiality and privacy of clients and families.
  • Annually access the training and education on patient, client and personal safety made available by Nova Scotia Health
  • Competencies in other languages an asset, French preferred
