This position provides a full range of social work services to clients and families, that encompass clinical social work; counseling; social work consultation and education services for Palliative Care clients and families in community setting. The Palliative Care Social Worker will act as an advocate for those with a life-limiting conditions assuring basic human needs are in place while respecting the client’s right to self-determination while upholding human rights. The Palliative Care Community Social Worker will be an active member of the collaborative inter-professional Palliative Care Consultation Team and will work in shared care with each client’s Primary Health Care Team.Qualifications
