Reporting to the Provincial Manager, Continuing Care Placement, the Placement Coordinator liaises with home care, acute care, and long term care facilities to facilitate appropriate and effective long term care placements. The Placement Coordinator receives long term care assessments for clients who are referred for Placement, communicates with client/Substitute Decision Maker on placement issues, manages the waitlist for Long Term Care ensuring that clients are admitted to long term care as per the Department of Seniors and Long Term Care policy, coordinates admissions to Long Term Care facilities; coordinates transfers of residents between facilities; maintains records and provides written and statistical reports as identified by Nova Scotia Health.
