Job Description

Explore our amazing new opportunities.Summary:The Program Coordinator leads a team of Case Managers to achieve program outcomes, and is responsible to provide daily operation support, such as client recruitment, researching practical resources, assisting in-group facilitation, providing emotional support/debriefing, accompanying Case Managers to attend home visits, and compile narrative reports and statistics as required.Employment: Temporary Full-Time (October 31, 2022 to August 31, 2023Reports to: Regional ManagerKey Duties & Responsibilities:

  • Provides support to ACT Case Managers on recruitment through outreaching to at-risk/vulnerable immigrant and refugee families/youth
  • Supports ACT Case Managers on conducting comprehensive intake, eligibility and needs assessment
  • Provides support to ACT Case Managers on wraparound meetings
  • Provides support to ACT Case Managers on conducting home visits when necessary
  • Liaises and works closely with community agencies and partnering agencies
  • Provides support to ACT Case Managers on conducting life skills, literacy and essential skills group sessions for clients
  • Provides daily practical resources and support to ACT Case Managers
  • Facilitates crisis interventions to effectively address immediate crisis situations
  • Debrief with ACT Case Managers on crisis situations and/or clients’ situation
  • Work collaboratively with other ACT Program Coordinators
  • Provides statistical and narrative information for the Regional Manager to complete the reports
  • Ensures all services meet the accreditation standards
  • Cover ACT Case Managers’ tasks when ACT Case Manager is absence
  • Ensure ACT Case Managers maintain up-to-date case record and group record
  • Maintains statistical records for accountability, funding application/report and program development purposes
  • Attends case conferences, supervision and team meetings regularly
  • Attends division staff meeting and training opportunities as required
  • Works closely with staff members of other S.U.C.C.E.S.S. divisions/offices in providing joint support to individuals and families
  • Performs other tasks as assigned by Regional Manager
Qualifications & Experience:
  • Bachelor degree or Diploma in Social Sciences, Human Sciences or related discipline
  • Two (2) years of family/youth work experience
  • Demonstrated knowledge about local community resources for at-risk/vulnerable immigrant population
  • Experienced in conflict resolution, case management and basic counselling skills
  • Experienced in conducting workshops
  • Demonstrates strong understanding on case planning and case management principles and techniques
  • Demonstrates good knowledge of issues vulnerable immigrant families and youth are facing, including social, cultural, emotional, mental health, health and sexual orientation issues
  • Demonstrates language and cultural proficiency that is applicable to the various source countries of vulnerable populations in their service area
  • Demonstrates knowledge and experience in cross-cultural communication and service methods appropriate to clients with low literacy and low English skills
Job Skills & Abilities:
  • Excellent verbal and written communication skills in English with at least one second language to perform language-specific service
  • Excellent in interpersonal and communication skills
  • A good team player with internal and external stakeholders
  • Ability to deal effectively with people from diverse backgrounds
  • Ability to make sound judgements
  • Excellent program planning, time management and organizational skills
  • Proficiency in the use of Microsoft Office Suite and basic internet skills
  • Ability to perform home visits and outreach
  • Multitasking and flexible
Other Requirements:
  • Criminal Record Check Required – Vulnerable Sector
  • Eligible to work in Canada
  • Flexible to work on evening, weekends and holidays
  • Need to travel occasionally
  • Need to attend to crisis intervention which may extend beyond the normal office hour occasionally
