Today, we are a strong, growing community of over 1,300 members. We live on a small portion of our traditional territory, known as the Musqueam Indian Reserve, located south of Marine Drive near the mouth of the Fraser River. The name Musqueam relates back to the River Plant, the name of the plant is məθkʷəy̓. There is a story that has been passed on from generation to generation that explains how we became known as the xʷməθkʷəy̓əm (Musqueam) - People of the River Plant. We have always moved throughout our territory using the resources it provides for fishing, hunting, trapping and gathering. We remain distinct and our cultural practices are strong, despite the devastating impacts of residential schools, colonial laws banning our ceremonies, and other attempts to assimilate our people. Our lands and waters continue to support our cultural and economic practices while serving as a source of knowledge and memory, encoded with our teachings and laws. To learn more about Musqueam, please visit www.musqueam.bc.caFull-Time 3-Month Term PositionPOSITION SUMMARYThe Community Health Representative and Liaison (CHR/L) is responsible for participating in the delivery of high quality health education to the Musqueam community, supporting the Musqueam Health Department staff in the implementation and coordination of health programs on reserve, and liaising between patients, families, the community, and health care authorities to ensure a high quality of care. The CHR/L also provides resources and support to the Musqueam community regarding COVID-19, through coordinating with the regional health authority (RHA) contact tracers to ensure culturally safe practices. This role provides vital prevention and public health related support, specifically in relation to COVID-19 pandemic.PRINCIPAL DUTIES AND RESPONSIBILITIESSupports the coordination and implementation of Health Department programs affected by the pandemic and works with CHN, Primary Care Clinic, and health staff to promote health updates and services (i.e. COVID restriction updates, vaccine distribution, mental health services, NNADAP, etc.)Demonstrates a healthy lifestyle and is a role model for the community.Monitors the community for general health, including during epidemics and emergencies, by supporting CHN, Home Care, etc.Works collaboratively with Musqueam community, families, and RHA COVID-19 contact tracers to support culturally safe contact tracing services:Supports contact tracing process by monitoring designated community members via phone to ensure that holistic needs are being met and isolation plans are being maintained.Problem solves community specific barriers to support contact tracers – e.g. uses alternative methods of contacting individuals when one may not have a phone, etc.Ensures individuals and families understand COVID-19 precautions, developing specific procedures for the community with the CHN and sharing information on basic infection control practices to prevent any future transmission of the virus;Develops and maintains strong working partnerships with communities, health care providers, health system partners and relevant organizations to support coordinated care.Acts as a direct resource to community – answering or responding to questions of citizens directly.Connects to the communities’ away-from-home population/families in event of an outbreak.Assists in identifying accommodations in community that could be used, modified if needed, for self-isolation, and/or surge capacity.Assists community in ordering, receiving and distributing personal protective equipment.Provides regular reporting on services provided.Performs other related duties as assigned.JOB SPECIFICATIONSGrade 12 or equivalentDemonstrated ability to maintain confidentiality of sensitive informationAbility to organize and plan own workAbility to be flexible and meet changing prioritiesGood verbal and written skillsStrong ability to collaborate with others and work well in a teamAbility to keep clear notes and provide professional reports when requestedOther detailsPay Type Hourly
