Reporting to the Professional Practice Leader, Clinical Nutrition, the Clinical Specialist, Complex Nutrition Support functions as an expert practitioner, educator, consultant and researcher by applying a conceptual model and relevant theory at an advanced level to design, implement, evaluate and document the appropriate nutritional care plan of patients experiencing intestinal failure requiring prolonged and complex nutrition support. Participates in the planning and daily operation of the Provincial Home Parenteral and Enteral Nutrition Programs (Home Nutrition Support Team). Initiates and implements research projects and educational programs. Provides recommendations for supplies contracts by monitoring program costs from a clinical needs perspective. Maintains certification as a Certified Nutrition Support Clinician (CNSC) from the American Society of Parenteral and Enteral Nutrition (ASPEN). Dietetic practice education from an accredited program meeting the Integrated Competencies for Dietetic Education and Practice (ICDEP). Current registration in the College of Dietitians of British Columbia with Restricted Activities A, B and C.DutiesProvides clinical leadership and consultation for other dietitians and health care professionals practicing by providing support for handling complex nutrition support and intestinal failure cases and acting as a resource provincially and nationally.Using expert skills, provides comprehensive nutritional assessment for patients with intestinal failure requiring prolonged and complex nutrition support; performs physical nutrition assessments; designs, implements, evaluates and documents an appropriate nutritional care plan; makes changes to nutritional care plans as necessary; liaises with health care providers and care team members for patients.Instructs and/or counsels patients and family members, other health care providers in the nutrition management of intestinal failure including parenteral, enteral and oral nutrition support interventions.Collaborates with the multi-disciplinary Home Nutrition Support Team to review and provide follow-up, for enteral and parenteral patients receiving nutrition support at home throughout the province.Determines, refers and/or liaises with acute and community based nutritional resources as required to ensure that home enteral and parenteral patient care issues are addressed. Completes requests to the Ministry of Health for funding for special products for patients as necessaryAssists Home Nutrition Support Team leadership in the fiscal management of the program by reviewing patient cases to make recommendations for optimal product usage. Provides recommendations for supplies contracts and participates in monitoring program costs from a clinical needs perspective.Initiates, organizes, and participates in education activities by collaborating with Clinical Nutrition and Home Nutrition Support team members and others to organize and give in-service education for dietitians and other health care professionals throughout the province. Further collaborates regarding the need for educational programs including participating in preparing educational materials, and developing and presenting educational programs for health care professionals.Initiates, organizes and participates in quality improvement activities by planning, developing, implementing, and evaluating written procedures and standards, assessment forms and patient materials, program descriptions, audit tools and clinical guidelines for complex and prolonged nutrition support to be used by Clinical Nutrition and the Home Nutrition Support team, dietitians and other health care providers throughout the province.Initiates, organizes and participates in research activities by identifying areas of research, writing proposals, implementing, evaluating projects and surveys, within departmental resources; collaborates with team to disseminate findings and apply findings to practice. Participates in the National Home Parenteral Nutrition (HTPN) Registry.Identifies nutrition related patient educational needs related to intestinal failure and complex and prolonged nutrition support; initiates, prepares and presents educational programs and materials in collaboration with departmental, program and other team members.Assists with practice education for dietetics students and other health care professionals by identifying learning objectives and enabling activities, and providing clinical education and supervision in the area.Participates in projects and committees and attends meetings as required.Completes required patient/resident documentation, forms and reports and documents workload statistics.Monitors and controls levels of supplies and equipment.
