Together with our lands, our customs and traditions are the foundations of who we are as Skwxwú7meshABOUT THE OPPORTUNITYReporting to the Manager of the Community Health and Wellness division, the Social Worker is a registered and experienced clinician who is a valued member of our community-based interdisciplinary health and wellness team. They practice from a place of respect for, and a willingness to learn about, the Squamish worldview that recognizes and honours the values, beliefs and customs of individuals, families and the community, as well as adhering to best practices current to the field of Social Work. The incumbent collaborates effectively and creatively with the client, their family, team members, our Elder Advisor, and partners to assess client need, and provide and/or coordinate holistic wraparound care for concurrent disorders. The Social Worker provides crisis/short-term counselling to help stabilize the individual, and then refers out to longer-term counselling. They place an emphasis on outreach, liaise with other Squamish Nation, partner Nation, and community supports, help the client to navigate the relevant systems, and provide advocacy where necessary.Typical Duties & Responsibilities:Provides clinical assessment and treatment planning, case formulation, goal setting, promotion of engagement and progress evaluation, prevention strategies and the sourcing of alternative or traditional healing methods to community members.Provides education and short-term individual and group therapy maintaining non-judgmental support relationships with clients including harm reduction approaches and cultural safety.Provides outreach services to community members in need and finds creative and effective ways to connect with those who are more difficult-to-reach (in both the Squamish Valley and on the North Shore).Provides intake for mental health and substance use, developing a Wellness Plan with community members.Works collaboratively with the client and their family, as well as with the Yúustway Health and Wellness team, other relevant Squamish Nation departments, and other referral sources to provide treatment-planning information.Provides crisis counselling and consultation (as needed), including suicide risk assessment, family assessment and mental health assessment.Actively participates in interdisciplinary case management meetings for management of client mental health and substance use issues.Participates in the coordination of permanent or temporary housing supports for clients (including for those who have been discharged from hospital).Supports education and coordination of health and community programs and services by participating, developing and providing education services related to concurrent disorders.Effectively liaises, and maintains professional relationships, with Yúustway’s partner agencies.Works with the Community Health and Wellness team to assess community needs to inform program/project planning, development, and implementation.Plans, implements and supports Community Health and Wellness and departmental community events, groups and activities.Documents accurate program and client records in our Electronic Medical Records system while maintaining strict confidentiality and privacy of information.Maintains regular communication with the Community Health and Wellness Manager and provides regular documentation and reporting to them, including quarterly reports to the funder and ensuring any challenges, barriers, service gaps and program development needs are communicated.Contributes cooperatively to the Squamish Nation Yúustway Health and Wellness team to achieve position and department goals.Maintains current knowledge and skills within the clinical field.Adheres to workplace health and safety policies, safe work practices and procedures, particularly while working in the field.Serves on the Suicide Prevention and Intervention Committee, and other relevant committees where required, and reports back to the CHW team.Performs additional duties as required.Education, Level of Experience & Hiring Requirements:Master’s degree in Social Work, or related discipline, from a recognized post-secondary institution.Registered with the BC College of Social Workers.3-5 years of directly related experience in the majority of duties listed in the job description including social work, outreach and navigation, resource and liaising work, caseload management, counselling and, ideally, Squamish Nation cultural knowledge.3 plus years of recent related social work and/or therapeutic clinical counselling experience within a concurrent disorder, substance use or mental health educational program.Recent (in the last 3 years) Indigenous Cultural Safety training, or willingness to complete it.Experience as an intermediate level user of the Internet and Microsoft Office (including Word, Excel, PowerPoint, Outlook, etc.)Knowledge of computer-based data management programs and information systems, such as an EMR (Electronic Medical Record), or ability to learn.Experience working in a First Nations environment is an asset.Must be able to work evenings and weekends as required.
