Job Description

QUALIFICATIONS

  • Post secondary education in a health related field
  • Two (2) years of professional experience in a Community Health Care setting
  • Province of Manitoba Class 5 Drivers License, and access to a personal vehicle to provide service within Prairie Mountain Health
  • Experience, knowledge and demonstrated ability to work with childbearing women/families living in at risk situations
  • Group facilitation skills and experience
  • Demonstrated application of a strength based and harm reduction approach
  • Must be competent with Windows based programs (Microsoft Word, Excel, PowerPoint, and Outlook)
  • Physically capable of providing service in a variety of community settings under varying weather and environmental conditions
  • Ability to establish and maintain positive working relationships with co-workers, clients and manager
  • Demonstrated flexibility to facilitate changes in techniques and procedures
  • Demonstrated leadership
  • Demonstrated written and oral communication skills
  • Ability to prioritize in a changing environment

POSITION SUMMARYThe role of the Healthy Baby Facilitator is to plan, promote, and implement programming, in accordance with Healthy Child Manitoba-Healthy Baby and Prairie Mountain Health vision and mission. Healthy Baby programs are intended to reach and support pregnant women and families with babies up to the age of one, with the primary focus on those whose needs may be overlooked or under-served by mainstream health services. This includes pregnant women/girls and new parents and babies who live on low incomes or who may be isolated, vulnerable or “at-risk" for a variety of reasons. Programs may attract those individuals who would simply benefit from connecting with other pregnant women and new parents. Programs will be primarily designed for groups, be family centered and strength based. The facilitator will offer social support and informal learning opportunities to encourage early, regular prenatal care and promote healthy infant development. The facilitator works in collaboration with individuals, families, communities and professionals.RESPONSIBILITIES:Overview:
  • Ensure programs are planned and delivered according to Healthy Child Manitoba Healthy Baby Community Program Guide, Healthy Baby Data Collection Guide and Prairie Mountain Health policies and procedures.
  • Work in a collaborative manner with program team to provide coordinated, quality community services for pregnant women and families with new babies.
  • Maintain knowledge base of current evidence based practice as it relates to Healthy Baby programming.
  • Manage the Healthy Child Manitoba Milk Program for assigned communities and Healthy Baby Benefit referrals.
  • Support eligible women to access the Manitoba Healthy Baby benefit and ensure knowledge and awareness of all available resources.
  • Build parents’ knowledge and awareness in the areas of nutrition, health, infant development and attachment.
  • Encourage healthy eating through nutrition activities and information.
  • Promote breastfeeding, health and healthy life style choices by supporting parents during pregnancy and their child’s first year of life.
  • Engage and support pregnant women and new parents who may be isolated and/or disadvantaged due to low income or other reasons.
  • Support strong parent-infant and family relationships.
  • Participate in the Healthy Baby evaluation through timely submission of all relevant forms.
  • Comply with required administrative duties.
  • Sundry duties as assigned.
