Implement therapeutic interventions for chronic, stable, common conditions. Utilizeamp; synthesize appropriate health assessmentamp; differential diagnosis of client/patient status. Assess need for orderingamp; interpreting appropriate laboratory, radiology and ultrasound tests. Work with client, family and health providers to developamp; implement holistic treatment, interventionsamp; support. Provide educational support for students. Support and participate in multi-level chronic disease management strategies. Assist individuals, familiesamp; communities to increase knowledge, controlamp; influence of health determinants in the maintenanceamp; improvement of their health status. Work with health care, community and inter-sectoral partners. Supportamp; assist with development/implementation of quality initiatives to benefit service delivery.Human Resources Exemption: NoExperience

• Three (3) years full-time experience as a Registered Nurse

Education

• Bachelor of Science in Nursing (BScN)

Licenses

• Valid Class 5 driver #39;s license

Other Education and Training

• Basic Life Support Certificate (BLS)
