Your Opportunity:Self Managed Care (SMC) is a service delivery option offered by the Home Care Program to Home Care Clients. Funding is provided to clients based on assessed needs as determined by Case Managers trained to complete SMC Assessments. If you’re a leader inspired by helping seniors live at home for as long as possible, this is the opportunity for you! The Self Managed Care Lead is an important role. Reporting to the Sr. Practice Consultant, Seniors Health, you are responsible for Self Managed Care (SMC) service delivery in the North Zone. You will oversee the Self Managed Care program, facilitate working groups, and incorporate ongoing review and continuous quality improvement into the program. You will communicate with a variety of health care providers across all care sectors, within Seniors Health and multi-disciplinary departments. You will provide education and consultation to the zone health care team, consult with Case Managers, and liaise with Finance, Seniors Health, and Home Care Operations. This position is based in North Zone – Location is NegotiableDescription:Provide Zone oversight of Self-Managed Care Program, including facilitation and consensus building of North Zone review panel, consultation with Case Managers, and liaisons with Finance Departments, Provincial Seniors Health Teams and Home Care Operations. Deliver Zone wide Self-Managed Care Education annually and on-going. Develop and maintain strong working relationships with key contacts in AHS, with North Zone Operations, AHS Seniors Health Teams and colleagues in other AHS Zones. Lead and participate on working groups and committees. Contribute to program development that supports the service delivery model, guiding principles and vision for Seniors Health. Be adaptable to changing workload and priorities. May also include providing project support to other client directed care service models.
