Job Description

EMPLOYMENT OPPORTUNITYPOSITION: HOME SUPPORT WORKER – LEVEL TWOJOB SUMMARY: Reporting to the Home Care Nurse, the Home Support Worker – Level Two, assists community members to live independently and safely in their own homes and community while enjoying the highest quality of life possible.GENERAL DUTIES:

  • Provides personal care services under the supervision of the Home Care Nurse
  • Performs delegated and client specific health care tasks
  • Aids with activities of daily living
  • Provides information and education to clients and family members regarding home management and housekeeping techniques to assist the client and family with self-care
  • Accompanies clients to community events where appropriate
    • Assists clients in making and keeping appointments with various health services
  • Reports changes in the health status, behaviour or safety of living situation of clients
  • Maintains and submits timely, accurate records of all services provided to clients
  • Documents and records according to professional and legal obligations
  • Participates in team meetings and as a member of a multidisciplinary team
  • Other related duties as may be required
QUALIFICATIONS:
  • Grade 12
  • Certification in Home Support/Residential or Personal Care
  • A minimum of one year of experience working directly with Elders in a residential or home care setting, preferably in a First Nations environment
  • First Aid and Food Safe certification
  • Must undergo a Criminal Records Check and Vulnerable Sector Search
