Job Description

Job DescriptionThe Advanced Practice Practitioner (APP) delivers expert-level clinical practice within an interprofessional model of care. The APP working in this role will have acquired an expert knowledge base in orthopedics and rheumatology. They will have additional formal training and possess sound decision-making skills and clinical competencies for expanded scope of practice. The APP is a champion for best practice, and innovative care models and contributes to quality improvement initiatives to achieve safe, effective, efficient care.The APP in this role has the necessary skills, competencies and complex decision-making capacity to address the needs of patients presenting with a range of MSK and Rheumatoid conditions. In this position, the EAC APP will work in an independent and interdependent relationship with the Rheumatologist, which will allow for effective collaboration and consultation. The APP will triage, prioritize, evaluate and make appropriate clinical decisions in consultation with the Rheumatologist.Accountabilities:

  • Applying advanced clinical skills to perform autonomous comprehensive assessments that incorporate results of the clinical history, physical examination, imaging and standardized outcome measurement
  • Applying advanced knowledge, skills, and attitudes enabling them to address complex problems and manage risk in unpredictable contexts
  • Applying current knowledge and advanced skills to assess, investigate, diagnose, and manage patients with MSK and Rheumatoid conditions referred to the program
  • Working under medical directives to order appropriate imaging and laboratory tests to support assessment, diagnosis and decision-making
  • Providing education and recommendations to patients, families and their primary care providers to support the management of their condition and optimize function in keeping with best practice guidelines
  • Effectively identifying, evaluating, and addressing disease prevention and health promotion issues of the population in the practice while administering quality patient care
  • Responsible for leadership and administrative duties related to EAC and research studies as needed
  • Contributing to program evaluation and research in the development, evaluation and improvement of team-based care and practice innovations
  • Participating in research to address problems and issues arising from practice
  • Active leadership and consultancy to support practices and care delivery
  • Promoting a positive working environment and enhances staff development
  • Acting as an ambassador/lead for the program as required
