Your Opportunity:The Integration & Innovation division of the Primary Health Care (PHC) program is a passionate and diverse team of system transformers that work towards integrating primary health care within Alberta’s provincial health system. We partner to design and implement new or improved ways of delivering health care that work for people in the community. By establishing and leveraging shared working structures with stakeholders at the zone and provincial levels and across organizations we support complex system initiatives. We coordinate the implementation of provincial primary health care integration priorities by bringing expertise in access and quality improvement, behavior change management and patient centered practices and collaborating with others with similar expertise across the system. We meaningfully engage patients and families as true partners in co-design to shape the work of and help transform the health care system.Description:Do you thrive on working closely with providers, patients and family advisors to create a better health care system Do you want to work with a team to further develop patient informed practices Are you ready to apply your skills in communications, facilitation, building relationships and managing project work The Consultant will work in a collaborative team culture within the Alberta Surgical Initiative supporting projects that improve access and quality of pre and post-surgical community supports. In addition, this role will work as part of a dyad in the provincial coordination of the Better Choices, Better Health® program offered at the zone level – supporting the promotion of self-management skills to Albertans living with chronic conditions. The position works to ensure the values, goals and principles of Alberta Health Services are implemented and integrated in the delivery of services.
