Job Description

Your Opportunity:The Health Promotion Facilitator II (HPF II) works with a high degree of independence on the Community Health Promotion (CHP) team, and under the direction of the Manager and/or Team Lead. The HPF II is responsible for planning, developing, implementing, and evaluating health promotion and disease prevention projects and initiatives using a population health approach. This position collaborates with diverse stakeholders, including internal staff and departments, government, community partners, and external agencies to support optimal health and well-being of individuals, groups, communities, and populations with a focus on chronic disease prevention initiatives. The current focus of this role pertains to community health promotion and chronic disease prevention. As a member of an interdisciplinary, provincially focused team, the HPF II utilizes a high level of research, evaluation, knowledge translation, and project management skills in concert with their knowledge of health promotion strategies including creating supportive environments, developing personal skills, strengthening community action, building healthy public policy and reorienting health services, to initiate, develop, implement, monitor, and evaluate health promotion initiatives at the community, local, and provincial levels. The initiatives may be housed within the CHP team or other Promoting Health/Provincial Population and Public Health teams.Description:As a Health Promotion Facilitator II, you will lead and coordinate health promotion, disease and injury prevention initiatives and programs using a population health promotion approach. You will identify behavioural, social, political, economic, environmental and organizational factors that promote or compromise health and recommend evidence-informed strategies to address identified priorities. You will conduct situational analyses, literature reviews, environmental scans and focus groups to make recommendations as part of an integrated program planning and evaluation process. You will manage projects using standardized tools and techniques that are conducive or aligned with health promotion, disease and injury prevention practices. You will collaborate with key internal and external key stakeholders in project or program planning, development, implementation, evaluation and improvement. You will facilitate community and staff engagement and build community and staff capacity around population health promotion priorities. You will evaluate the effectiveness of existing health promotion and disease prevention strategies and make recommendations.

Required Qualifications:Completion of masters degree in Health Promotion, Public or Population Health, Health Education or a related field.Additional Required Qualifications:Minimum three (3) to five (5) years’ experience in relevant health promotion or population/public health or community based experience or mental health and addictions. Extensive experience in developing frameworks, reports, plans (e.g., business, work, project, program strategic, action, or implementation plans) and other related products (e.g., literature reviews, situational and needs analyses, resources and tools, logic models, and grant/funding proposals.) Community development experience, including ability to conduct community assessments, hold community consultations, and develop community capacity. Demonstrated organizational skills and ability to work both independently and collaboratively with a variety of disciplines, sectors, and internal and external stakeholders/partners. Demonstrated skills in developing and applying creative approaches to program and project planning. Demonstrated critical thinking skills, including conceptualizing, strategizing, analyzing, reflecting, and problem-solving; must be able to view situations from different points of views and from different levels. Demonstrated knowledge of various research and evaluation methods. Demonstrated strong communication (written, verbal, and interpersonal) skills for a variety of purposes and audiences. Group facilitation, chairing meetings, teaching, and presenting. Effective time management and organizational skills.Preferred Qualifications:Experience, knowledge and expertise in chronic disease prevention, risk reduction, and health promotion in the adult and senior populations is an asset.
