Job Description

Your Opportunity:Reporting to the Manager of Addiction and Mental Health (AMH) Regional Housing, the Outreach Worker I works as a member of the AMH team at Templemont Gardens Supportive Living Level 4 (SL4) Facility; collaboratively working with staff from Integrated Supportive and Facility Living (ISFL) and Trinity Place Foundation of Alberta (TPFA) to support adults assessed for SL4 level of care with identified complex physical, mental or cognitive impairments who may also have a history of substance use disorder. The AMH team is comprised of Nursing, Social Work, Outreach Workers, Addictions Counsellors and a Psychiatrist. This team provides assessment, treatment, intervention, psychosocial rehabilitation, addiction support services and case management functions to the clients at Templemont Gardens. As the Outreach Worker, your role is to support the clients to meet the goals on their individual treatment plans. Role responsibilities include supporting clients with ADLs (activities of daily living) and iADLs (instrumental activities of daily living) training, participating in recreation/leisure and other group therapy/individual activities on site and in the community. You will also facilitate client connections with community/government supports and advocate for clients with these external service providers.Description:As an Outreach Worker, you will work as a member of a multidisciplinary team, supporting clients to stabilize in their current environments as an extension of their clinical care service. As an Outreach Worker, you will conduct outreach assessments, determine outreach service goals and develop outreach treatment plans in collaboration with clients and clinicians. You will facilitate client connections with community and government supports and advocate for clients with these external service providers. You will focus on establishing housing, financial, food and clothing, transportation and social supports for clients. You will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.), observing and reporting changes in the clients behaviours, participation and ability to function independently to the interdisciplinary team. You will evaluate client progress and functioning and make adjustments to the outreach treatment plan as required. As a part of your role as an Outreach Worker, you may be required to provide outreach crisis stabilization support for clients, exercising various de-escalation techniques and linking clients with the appropriate supports (EMS, Police and Crisis Team, etc.). You also may be required to enter clients residences and assist clients to transfer in and out of vehicles, care facilities and residences.

Required Qualifications:Completion of diploma from a post-secondary program in a related field.Additional Required Qualifications:Minimum 2 years experience within the last 3 years working with adults with severe and persistent mental health and addiction issues. Recent community-based mental health outreach experience. Experience working with multidisciplinary teams and community agencies. Knowledge of relevant community resources and legislation Ability to be self-directed and exercises independent decision-making, and critical thinking skills. High degree of flexibility as well as the ability to work independently in an outreach capacity. Ability to work in stressful, demanding and changing conditions. Excellent interpersonal and communication skills with the ability to effectively maintain collaborative working relationships with members of the multidisciplinary team. Excellent time management and prioritization skills, with a demonstrated ability to function effectively within a dynamic environment. Demonstrated problem solving and conflict resolution skills. Actively integrates patient and family centered care principles within own practice standards. Excellent verbal and written communication skills. Basic knowledge of computers and software applications (i.e. Microsoft Office).Preferred Qualifications:Experience in conducting psychosocial assessments, treatment planning and crisis intervention.
