Please note this applies to all postings, and individual medical exemptions must be approved by the Provincial Health Officer.As a member of a multidisciplinary team, provides direct nursing care to patients of the Hospice Residence at assigned locations according to established policies and procedures by assessing physical, emotional, spiritual and psychosocial needs; plans, documents and implements care plans; evaluates the effectiveness of the care plan and makes recommendations for care plan changes, where necessary.- Assesses, plans, implements and evaluates nursing care of assigned patients by assessing actual or potential problems and strengths, plans and performs interventions and evaluates outcomes.- Collects information about the patient using a variety of sources such as skills of observation, communication and physical assessment to enable appropriate nursing care.- Communicates and consults with members of the health care team about the patient care plan; participates in interdisciplinary conference and explains plan of care to the patient and/or families.- Acts as an advocate to protect and promote the patient #39;s rights to autonomy, respect, privacy, dignity and access to information.- Documents timely and accurate reports of relevant observations including patient and family teaching and evaluation of nursing care.- Participates on nursing, hospital committees and councils for improvement of patient care and/or work environment.- Participates in the orientation and ongoing education of nursing staff and students by providing information and acting as a preceptor, where appropriate; identifies self-learning needs and attends educational programs to maintain and enhance clinical competency.- Participates in quality improvement activities by identifying patient care issues and collecting data, evaluating patient care programs, techniques, standards and procedures by providing feedback about effectiveness and patient care outcomes.- Maintains a safe environment for patients, families and staff by adhering to established safety and emergency practices and procedures.- Performs other related duties as assigned.Education and ExperienceGraduation of an approved School of Nursing and/or Psychiatric Nursing.
